摘要:
目的:探讨扫描式葡萄糖监测(FGM)系统在妊娠期糖尿病患者血糖管理中的应用。方法:选取妊娠期糖尿病患者60例,随机分为两组,各30例,观察组采用FGM进行为期14 d的血糖监测,对照组采用指尖血糖监测,每天至少进行4次监测; 比较两组患者血糖达标时间、空腹血糖(FPG)、餐后2 h血糖(2hPG)、糖化血红蛋白(HbA1c)、低血糖发生情况、葡萄糖目标范围内时间(TIR)及妊娠结局情况。结果:两组孕妇血糖达标时间、FPG、2hPG、低血糖发生情况以及TIR比较差异均有统计学意义(均P<0.05),而两组糖化血红蛋白水平比较无统计学差异(P>0.05); 两组羊水异常、胎膜早破、胎盘早剥、早产、新生儿呼吸窘迫症、新生儿低血糖发生情况比较差异均无统计学意义(P>0.05),观察组孕妇的剖宫产率低于对照组(P< 0.05)。结论:FGM可以使妊娠期糖尿病孕妇血糖控制更加及时有效,降低孕妇的剖宫产率。
Abstract:
Objective:To investigate the application of flash glucose monitoring(FGM)in blood glucose management of patients with gestational diabetes mellitus.Methods:60 pregnant women with gestational diabetes mellitus were randomly divided into two groups,30 cases in each group.The observation group used FGM for 14-day blood glucose monitoring.The control group used finger tip blood glucose monitoring,at least 4 times a day.The time of blood glucose reaching the standard,fasting blood glucose(FPG),2-hour postprandial blood glucose(2hPG),glycosylated hemoglobin(HbA1c),incidence of hypoglycemia,time in range(TIR)and pregnancy outcome were compared between the two groups.Results:There were significant differences in the time of blood glucose reaching the standard,FPG,2hPG,incidence of hypoglycemia and TIR between the two groups(all P<0.05),but there was no significant difference in the level of HbA1c between the two groups(P>0.05).There were no significant differences in the incidence of abnormal amniotic fluid,premature rupture of membranes,placental abruption,premature delivery,neonatal respiratory distress and neonatal hypoglycemia between the two groups(all P>0.05).The cesarean section rate of the observation group was lower than that of the control group(P<0.05).Conclusion:FGM can make blood glucose control of pregnant women with gestational diabetes mellitus more timely and effective,and reduce the cesarean section rate of pregnant women.
