摘要:
目的:探讨iRoot BP Plus应用于年轻恒牙血运重建术对牙根发育及龈沟液血管内皮生长因子(VEGF)、碱性成纤维细胞生长因子(bFGF)、白细胞介素-8(IL-8)、白细胞介素-1β(IL-1β)水平的影响。方法:选取88例年轻恒牙血运重建术患儿作为研究对象,按照随机数字表法将其分为对照组(44例,44颗牙)和试验组(44例,44颗牙)。对照组使用无机三氧化物聚合物(MTA)作为填充材料,试验组选择iRoot BP Plus作为填充材料,两组术后均随访1年。比较两组随访1年后的疗效、牙根发育情况,术前、随访1年后的牙根长度、根管壁厚度、咬合功能、咀嚼功能、疼痛程度、牙周指数,术前、术后1周的龈沟液VEGF、bFGF、IL-8、IL-1β水平及随访期间的不良反应发生情况。结果:随访1年后,试验组总有效率及牙根发育Ⅰ型患儿占比分别为95.45%、47.73%,高于对照组的79.55%、25.00%(均P<0.05)。与术前比较,随访1年后,两组牙根长度、根管壁厚度增加,试验组高于对照组(均P<0.05); 两组咬合功能、咀嚼功能、视觉模拟评分法(VAS)、牙周袋深度(PD)、牙龈指数(GI)、龈沟出血指数(SBI)、菌斑指数(PLI)评分降低,试验组低于对照组(均P<0.05)。与术前比较,术后1周,两组龈沟液VEGF、bFGF水平升高,试验组高于对照组(均P<0.05); 两组龈沟液IL-8、IL-1β水平降低,试验组低于对照组(均P<0.05)。随访期间,试验组不良反应总发生率为4.55%,低于对照组的22.73%(P<0.05)。结论:与MTA比较,iRoot BP Plus应用于年轻恒牙血运重建术中可调节患儿龈沟液VEGF、bFGF、IL-8、IL-1β水平,减轻炎症,促进牙根生长发育,并可减轻患儿疼痛程度,改善咬合功能、咀嚼功能及牙周状况,进而有利于提高疗效,且具有较好的安全性。
Abstract:
Objective:To investigate the effect of iRoot BP Plus application in young permanent teeth revascularization surgery on root development and levels of vascular endothelial growth factor(VEGF),basic fibroblast growth factor(bFGF),interleukin-8(IL-8),and interleukin-1β(L-1β)in gingival crevicular fluid.Methods:88 young permanent tooth revascularization children were selected as the study subjects and randomly divided into the control group(44 cases,44 teeth)and the experimental group(44 cases,44 teeth)using a random number table method.The control group used mineral trioxide aggregate(MTA)as the filling material,while the experimental group chose iRoot BP Plus as the filling material.Both groups were followed up for 1 year after surgery.The efficacy and root development after 1 year of followed up,the root length,root canal wall thickness,occlusal function,chewing function,level of pain,periodontal index before surgery and after 1 year of followed up,and the levels of VEGF,bFGF,IL-8,IL-1β in gingival crevicular fluid before and 1 week after surgery and incidence of complications during followed up of two groups were ompared.Results:After 1 year of followed up,the total effective rate and the proportion of children with type Ⅰ root development in the experimental group were 95.45% and 47.73% respectively,higher than the control group's 79.55% and 25.00%(all P<0.05).Compared with before surgery,after 1 year of followed up,the root length and root canal wall thickness of the two groups increased,with the experimental group being higher(all P<0.05); the scores of occlusal function,chewing function,visual analogue scale(VAS),periodontal pocket depth(PD),Gingival Index(GI),Sulcus Bleeding Index(SBI),and plaque index(PLI)decreased in both groups,with the experimental group showing lower(all P<0.05).Compared with before surgery,the levels of VEGF and bFGF in the gingival crevicular fluid of the two groups increased after 1 week after surgery,the experimental group was higher(all P<0.05); the levels of IL-8 and IL-1β in gingival crevicular fluid decreased in both groups,the experimental group was lower(all P<0.05).During the followed up period,the total incidence of complications in the experimental group was 4.55%,lower than the 22.73% in the control group(P<0.05).Conclusion:Compared with MTA,iRoot BP Plus could regulate the levels of VEGF,bFGF,IL-8,and IL-1β in the gingival crevicular fluid of young permanent teeth during revascularization surgery of childrens,reduce inflammation,promote root growth and development,reduce children pain,improve occlusal function,chewing function and periodontal condition,with safety.
