摘要:
目的:探讨血清固醇调节元件结合蛋白1(SREBP1)、Klotho与狼疮性肾炎(LN)患者疾病转归的关系。方法:选取112例LN患者,均接受标准化治疗后6个月,根据疾病转归情况分为好转组(78例)和恶化组(34例)。收集患者临床资料,采用ELISA法检测血清SREBP1、Klotho水平,免疫荧光法检测足细胞Nephrin、Podocin蛋白表达。Pearson法分析血清SREBP1、Klotho水平与患者足细胞Nephrin、Podocin蛋白表达水平的相关性。多因素Logistic回归分析LN患者疾病转归的影响因素。受试者工作特征(ROC)曲线分析血清SREBP1、Klotho对LN患者疾病转归的预测价值。结果:好转组基线肾小球滤过率(eGFR)高于恶化组,24 h尿蛋白定量、尿微量白蛋白低于恶化组(均P<0.05)。恶化组血清SREBP1水平高于好转组,Klotho水平低于好转组(均P<0.05)。恶化组Nephrin、Podocin蛋白表达水平低于好转组(均P<0.05)。血清SREBP1水平与患者足细胞Nephrin、Podocin蛋白表达水平呈负相关,Klotho水平与Nephrin、Podocin蛋白表达水平呈正相关(均P<0.05)。基线eGFR、24 h尿蛋白定量、尿微量白蛋白、足细胞Nephrin、Podocin蛋白及血清SREBP1、Klotho是LN患者疾病转归的独立影响因素(均P<0.05)。血清SREBP1、Klotho两项指标联合预测的LN患者治疗后疾病恶化的曲线下面积(AUC)为0.856,高于单一指标预测的AUC(均P<0.05)。结论:血清SREBP1、Klotho与LN患者足细胞损伤和疾病转归有关,两者联合对患者疾病转归的预测效能较好。
Abstract:
Objective:To explore the relationship between serum sterol regulatory element binding protein 1 (SREBP1),Klotho and disease outcome in patients with lupus nephritis (LN).Methods:A total of 112 patients with LN were enrolled.All patients received standardized treatment and were followed up for 6 months.According to disease outcome,the patients were divided into improvement group (78 cases) and deterioration group (34 cases).Clinical data of patients were collected.Serum levels of SREBP1 and Klotho were detected by ELISA,and the expression of podocyte Nephrin and Podocin proteins was determined by immunofluorescence assay.Pearson correlation analysis was used to analyze the correlations of serum SREBP1 and Klotho levels with the expression levels of podocyte Nephrin and Podocin proteins.Multivariate Logistic regression analysis was performed to identify influencing factors for disease outcome in LN patients.ROC curve analysis was adopted to evaluate the predictive value of serum SREBP1 and Klotho for disease outcome in LN patients.Results:The baseline estimated glomerular filtration rate (eGFR) in the improvement group was higher than that in the deterioration group,while 24-hour urinary protein quantification and urinary microalbumin were lower than those in the deterioration group (all P<0.05).The serum SREBP1 level was higher and Klotho level was lower in the deterioration group compared with the improvement group (all P<0.05).The expression levels of Nephrin and Podocin proteins in the deterioration group were significantly lower than those in the improvement group (all P<0.05).Serum SREBP1 level was negatively correlated with the expression levels of podocyte Nephrin and Podocin proteins,whereas Klotho level was positively correlated with Nephrin and Podocin expression (all P<0.05).Baseline eGFR,24-hour urinary protein quantification,urinary microalbumin,podocyte Nephrin and Podocin proteins,as well as serum SREBP1 and Klotho were independent influencing factors for disease outcome in LN patients (all P<0.05).The AUC of combined detection of serum SREBP1 and Klotho for predicting post-treatment deterioration in LN patients was 0.856,which was higher than the AUC of single indicator prediction (all P<0.05).Conclusion:Serum SREBP1 and Klotho are associated with podocyte injury and disease outcome in LN patients,and the combination of the two indicators presents favorable predictive efficacy for patients’ disease outcome.
