摘要:
目的:探讨磷酸肌醇3激酶调节亚基1(PIK3R1)、紧密连接蛋白4(CLAUDIN-4)和有丝分裂基因A(NIMA)相关激酶2(NEK2)在非小细胞肺癌(NSCLC)患者肿瘤组织中的表达水平及其诊断价值。方法:回顾性选取手术治疗的340例NSCLC患者为研究对象。分别采用RT-qPCR和Western blotting检测NSCLC组织与癌旁组织(距离肿瘤>3 cm)中PIK3R1/p85α、CLAUDIN-4和NEK2的mRNA和蛋白表达水平;卡方检验评估PIK3R1/p85α、CLAUDIN-4和NEK2蛋白水平与NSCLC患者临床病理特征的关系。此外,采用Pearson分析NSCLC患者肿瘤组织中PIK3R1/p85α、CLAUDIN-4和NEK2蛋白水平的相关性;采用受试者工作特征(ROC)曲线评估PIK3R1/p85α、CLAUDIN-4和NEK2水平对NSCLC的诊断效能。多因素Cox回归模型分析影响NSCLC患者预后的因素。结果:与癌旁组织比较,NSCLC患者肿瘤组织中PIK3R1的mRNA和蛋白水平显著降低,CLAUDIN-4和NEK2的mRNA和蛋白水平显著升高,差异具有统计学意义(均P<0.01)。此外,临床和病理参数的相关性分析显示,PIK3R1/p85α蛋白表达与NSCLC患者年龄、性别和病理类型无相关性(均P>0.05),与患者的肿瘤大小、临床分期、病理学分化、淋巴结转移和5年生存率呈显著负相关(均P<0.05);CLAUDIN-4蛋白表达与NSCLC患者年龄、性别和病理类型无相关性,与患者的肿瘤大小、临床分期、病理学分化、淋巴结转移和5年生存率显著正相关(均P<0.05);NEK2蛋白表达与NSCLC患者的肿瘤大小、病理类型、临床分期、病理学分化、淋巴结转移和5年生存率显著正相关(均P<0.05)。与此同时,PIK3R1/p85α、CLAUDIN-4和NEK2联合诊断NSCLC的曲线下面积(AUC)为0.995,敏感性为95%,特异性为100%,95%CI为0.988~1.000。Cox模型对NSCLC预后多因素生存分析结果显示PIK3R1/p85α、CLAUDIN-4和NEK2是NSCLC患者预后的独立危险因素,肿瘤大小、TNM分期、病理分级、淋巴结转移均是非独立危险因素。结论:PIK3R1/p85α、CLAUDIN-4和NEK2联合检测对NSCLC具有较高的诊断效能,值得临床推广应用。
Abstract:
Objective:To investigate the expression levels of Phosphoinositide-3-Kinase Regulatory Subunit 1 (PIK3R1),CLAUDIN-4 and Never in mitosis gene A (NIMA)-related kinase 2 (NEK2) in tumor tissues of non-small-cell lung carcinoma (NSCLC) patients and their diagnostic value.Methods:A retrospective study selected 340 patients with NSCLC who underwent surgery as the research objects.The mRNA and protein levels of PIK3R1/p85α,CLAUDIN-4 and NEK2 in NSCLC tissues and adjacent normal tissues (>3 cm away from the tumor) were detected with RT-qPCR and Western blotting;and the relationship between the protein levels of PIK3R1/p85α,CLAUDIN-4 and NEK2 and the clinical and pathological characteristics of NSCLC patients were analyzed by Chi square test.Furthermore,Pearson analysis was used to evaluate the correlation of PIK3R1/p85α,CLAUDIN-4 and NEK2 protein levels in tumor tissues of NSCLC patients,and the receiver operating characteristic (ROC) curve was used to evaluate the diagnostic efficacy of PIK3R1/p85α,CLAUDIN-4 and NEK2 levels in NSCLC.Multivariate survival analysis of NSCLC patients was performed using Cox model.Results:Compared with adjacent normal tissues,the mRNA and protein levels of PIK3R1/p85α in tumor tissues from NSCLC patients were significantly decreased,while CLAUDIN-4 and NEK2 mRNA and protein levels were significantly increased,with statistically significant differences (all P<0.01).Moreover,correlation analysis of clinical and pathological parameters showed that PIK3R1/p85α protein level was not correlated with age,gender,and pathological type of NSCLC patients (all P>0.05),but was significantly negatively correlated with tumor size,clinical stage,pathological differentiation,lymph node metastasis,and 5-year survival rate (all P<0.05);CLAUDIN-4 protein level was not correlated with age,gender,and pathological type of NSCLC patients,but was significantly positively correlated with tumor size,clinical stage,pathological differentiation,lymph node metastasis,and 5-year survival rate (all P<0.05);NEK2 protein level was significantly positively correlated with pathological type,tumor size,clinical stage,pathological differentiation,lymph node metastasis,and 5-year survival rate (all P<0.05).Meanwhile,the area under the curve (AUC) of PIK3R1/p85α,CLAUDIN-4 and NEK2 in the diagnosis of NSCLC was 0.995,the sensitivity was 95%,the specificity was 100%,and the 95%CI was 0.988~1.000.Multivariate survival analysis of the prognosis of NSCLC by Cox model showed that the protein levels of PIK3R1/p85α,CLAUDIN-4 and NEK2 were independent risk factors affecting the prognosis,while tumor size,TNM stage,pathological grade and lymph node metastasis were non independent risk factors.Conclusion:The combined detection of PIK3R1/p85α,CLAUDIN-4 and NEK2 levels has a high diagnostic efficacy for NSCLC,which is worthy of clinical application.
