摘要:
目的:探究妊娠期高血压疾病(HDP)患者血清血管生成素样蛋白8(ANGPTL8)、富含Glu/Asp的羧基末端结构域2(CITED2)、干扰素调节因子1(IRF1)水平变化及检测意义。方法:选取89例HDP患者作为病例组,同期进行产检的89例健康孕妇为对照组。根据病情进展情况将患者分为妊娠期高血压(GH)组(41例)、轻度子痫前期(PE)组(30例)、重度PE组(18例);根据妊娠结局将患者分为不良妊娠结局(APO)组(32例)和N-APO组(57例)。酶联免疫吸附试验检测血清ANGPTL8、CITED2、IRF1水平;多因素Logistic回归分析HDP患者发生APO的影响因素;受试者工作特征(ROC)曲线分析血清ANGPTL8、CITED2、IRF1对HDP患者发生APO的预测价值。结果:病例组血清ANGPTL8、CITED2、IRF1水平高于对照组(均P<0.05)。不同病情进展情况患者血清ANGPTL8、CITED2、IRF1水平比较,重度PE组高于轻度PE组,轻度PE组高于GH组(均P<0.05)。APO组舒张压、收缩压及血清ANGPTL8、CITED2、IRF1水平高于N-APO组(均P<0.05)。血清ANGPTL8、CITED2、IRF1是影响HDP患者发生APO的影响因素(均P<0.05)。血清ANGPTL8、CITED2、IRF1以及联合预测HDP患者发生APO的曲线下面积(AUC)分别为0.738、0.819、0.765、0.926,联合AUC高于血清ANGPTL8、CITED2、IRF1各自单独预测的AUC(均P<0.05)。结论:HDP患者血清ANGPTL8、CITED2、IRF1水平升高,与患者病情进展有关,三者联合检测对HDP患者发生APO有更高的预测效能。
Abstract:
Objective:To investigate the changes and detection significance of serum angiopoietin-like protein 8 (ANGPTL8),Cbp/p300-interacting transactivator with Glu/Asp-rich C-terminal domain 2 (CITED2),and interferon regulatory factor 1 (IRF1) levels in patients with hypertensive disorders of pregnancy (HDP).Methods:A total of 89 patients with HDP were selected as the case group,and 89 healthy pregnant women were selected as the control group.According to disease progression,patients were divided into gestational hypertension (GH) group (41 cases),mild preeclampsia (PE) group (30 cases),and severe PE group (18 cases).According to pregnancy outcomes,patients were divided into adverse pregnancy outcome (APO) group (32 cases) and non-APO (N-APO) group (57 cases).Serum ANGPTL8,CITED2 and IRF1 levels were detected by enzyme-linked immunosorbent assay.Multivariate logistic regression analysis was used to identify influencing factors for APO in HDP patients.ROC curve analysis was performed to evaluate the predictive value of serum ANGPTL8,CITED2 and IRF1 for APO in HDP patients.Results:Serum ANGPTL8,CITED2 and IRF1 levels in the case group were higher than those in the control group (all P<0.05).Comparison of serum ANGPTL8,CITED2 and IRF1 levels among patients with different disease progression showed that the severe PE group was higher than the mild PE group,and the mild PE group was higher than the GH group (all P<0.05).Diastolic blood pressure,systolic blood pressure,and serum ANGPTL8,CITED2 and IRF1 levels in the APO group were higher than those in the N-APO group (all P<0.05).Serum ANGPTL8,CITED2 and IRF1 were independent influencing factors for APO in HDP patients (all P<0.05).The AUC for serum ANGPTL8,CITED2,IRF1,and their combination in predicting APO in HDP patients were 0.738,0.819,0.765,and 0.926,respectively.The combined AUC was higher than that of serum ANGPTL8,CITED2 and IRF1 alone (all P<0.05).Conclusion:Serum ANGPTL8,CITED2 and IRF1 levels are elevated in HDP patients and are associated with disease progression.Combined detection of these three biomarkers demonstrates superior predictive efficacy for APO in HDP patients.
