摘要:
目的:探讨多囊卵巢综合征(PCOS)患者血清内脏脂肪特异性丝氨酸蛋白酶抑制因子(Vaspin)、前蛋白转化酶枯草溶菌素9(PCSK9)、葡萄糖转运蛋白4(GLUT4)水平变化及其与胰岛素抵抗(IR)的相关性。方法:选取PCOS患者100例为PCOS组,另选同期80例健康女性为对照组。根据胰岛素抵抗指数(HOMA-IR)将PCOS患者分为IR组(52例)和非IR组(48例)。比较各组血清Vaspin、PCSK9、GLUT4水平。Pearson法分析血清Vaspin、PCSK9、GLUT4水平与PCOS患者IR的相关性。结果:PCOS组血清Vaspin、PCSK9水平高于对照组,GLUT4水平低于对照组(均P<0.05)。IR组血清Vaspin、PCSK9水平较非IR组升高,GLUT4水平较非IR组下降(均P<0.05)。IR组体重指数(BMI)、空腹血糖、空腹胰岛素(FINS)、HOMA-IR、餐后2 h血糖(2hPG)高于非IR组(均P<0.05)。血清Vaspin、PCSK9水平与HOMA-IR、FINS、空腹血糖、BMI、2hPG呈正相关,血清GLUT4水平与上述指标呈负相关(均P<0.05)。结论:PCOS患者血清Vaspin、PCSK9水平升高,GLUT4水平降低,且三者与IR有关。
Abstract:
Objective:To investigate the changes in serum levels of visceral fat specific serine protease inhibitor (Vaspin),proprotein convertase subtilisin/kexin type 9 (PCSK9) and glucose transporter 4 (GLUT4) in patients with polycystic ovary syndrome (PCOS) and their correlation with insulin resistance (IR).Methods:A total of 100 patients with PCOS were enrolled as the PCOS group,and 80 healthy women were selected as the control group.According to the homeostatic model assessment for insulin resistance (HOMA-IR),PCOS patients were divided into an IR group (52 cases) and a non-IR group (48 cases).Serum levels of Vaspin,PCSK9 and GLUT4 were compared among the groups.Pearson correlation analysis was used to evaluate the correlation between serum Vaspin,PCSK9 and GLUT4 levels and IR in PCOS patients.Results:Serum levels of Vaspin and PCSK9 in the PCOS group were higher than those in the control group,while GLUT4 level was lower (all P<0.05).Compared with the non-IR group,the IR group showed increased serum Vaspin and PCSK9 levels and decreased GLUT4 level (all P<0.05).Body mass index (BMI),fasting plasma glucose,fasting insulin (FINS),HOMA-IR,and 2-hour postprandial glucose (2hPG) in the IR group were higher than those in the non-IR group (all P<0.05).Serum Vaspin and PCSK9 levels were positively correlated with HOMA-IR,FINS,fasting plasma glucose,BMI and 2hPG,whereas serum GLUT4 level was negatively correlated with these indicators (all P<0.05).Conclusion:Elevated serum Vaspin and PCSK9 levels and reduced GLUT4 level are observed in PCOS patients,and all three biomarkers are associated with IR.
