摘要:
目的:探讨男性不育症患者精浆生化指标、精子DNA碎片率与其体外受精-胚胎移植(IVF-ET)妊娠结局的关系。方法:选取88例男性不育症患者,行精液常规、精浆生化和精子DNA碎片率检测。患者及配偶接受IVF-ET助孕,随访至胚胎移植后第28~30天行超声检查,以观察到孕囊为临床妊娠判定标准。根据患者配偶妊娠结局分为妊娠成功组(40例)和妊娠失败组(48例)。比较两组患者及配偶基线资料、患者精液常规、精浆生化指标(精浆柠檬酸、中性α-葡糖苷酶、锌、果糖)、精子DNA碎片率。采用Logistic回归分析男性不育症患者IVF-ET妊娠结局的影响因素。采用Spearman法分析精浆生化指标、正常形态精子百分率及精子DNA碎片率与男性不育症患者IVF-ET妊娠结局的相关性。结果:两组患者及配偶资料比较差异无统计学意义(均P>0.05)。两组患者精液体积、前向运动精子百分率、精子浓度、精子活动率比较差异无统计学意义(均P>0.05)。妊娠成功组患者精浆柠檬酸、中性α-葡糖苷酶、锌、果糖水平及正常形态精子百分率高于妊娠失败组,精子DNA碎片率低于妊娠失败组(均P<0.05)。精浆柠檬酸、中性α-葡糖苷酶、锌、果糖及正常形态精子百分率、精子DNA碎片率为男性不育症患者IVF-ET妊娠结局的独立影响因素(均P<0.05)。精浆柠檬酸、中性α-葡糖苷酶、锌、果糖及正常形态精子百分率与患者IVF-ET妊娠结局呈正相关,精子DNA碎片率与患者IVF-ET妊娠结局呈负相关(均P<0.05)。结论:男性不育症患者精浆生化指标、精子DNA碎片率与其IVF-ET妊娠结局有关,是患者IVF-ET妊娠结局的影响因素。
Abstract:
Objective:To investigate the relationship between seminal plasma biochemical parameters,sperm DNA fragmentation index and in vitro fertilization and embryo transfer (IVF-ET) pregnancy outcomes in male infertility patients.Methods:A total of 88 male infertility patients were enrolled.Routine semen analysis,seminal plasma biochemistry,and sperm DNA fragmentation index were performed.Patients and their spouses underwent IVF-ET assisted reproductive treatment and were followed up until ultrasound examination at 28-30 days after embryo transfer,with the observation of gestational sacs serving as the criterion for clinical pregnancy.According to the pregnancy outcomes of the patients’ spouses,subjects were divided into the successful pregnancy group (40 cases) and the failed pregnancy group (48 cases).Baseline characteristics of patients and their spouses,routine semen parameters,seminal plasma biochemical indices (seminal plasma citric acid,neutral α-glucosidase,zinc and fructose),and sperm DNA fragmentation index were compared between the two groups.Logistic regression analysis was used to identify influencing factors for IVF-ET pregnancy outcomes in male infertility patients.Spearman correlation analysis was performed to analyze the correlations between seminal plasma biochemical parameters,percentage of morphologically normal sperm,sperm DNA fragmentation index,and IVF-ET pregnancy outcomes in male infertility patients.Results:No statistically significant differences were observed in baseline characteristics between the two groups of patients and their spouses (all P>0.05).No significant differences were found in semen volume,percentage of progressive motile sperm,sperm concentration,or sperm motility rate between the two groups (all P>0.05).The successful pregnancy group exhibited higher levels of seminal plasma citric acid,neutral α-glucosidase,zinc,and fructose,as well as a higher percentage of morphologically normal sperm,but a lower sperm DNA fragmentation index compared with the failed pregnancy group (all P<0.05).Seminal plasma citric acid,neutral α-glucosidase,zinc,fructose,percentage of morphologically normal sperm,and sperm DNA fragmentation rate were identified as independent influencing factors for IVF-ET pregnancy outcomes in male infertility patients (all P<0.05).Seminal plasma citric acid,neutral α-glucosidase,zinc,fructose,and percentage of morphologically normal sperm were positively correlated with IVF-ET pregnancy outcomes,whereas sperm DNA fragmentation index was negatively correlated with IVF-ET pregnancy outcomes (all P<0.05).Conclusion:Seminal plasma biochemical parameters and sperm DNA fragmentation index are associated with IVF-ET pregnancy outcomes in male infertility patients and serve as influencing factors for IVF-ET pregnancy outcomes.
