摘要:
目的:探讨急性缺血性脑卒中(AIS)介入后血清谷胱甘肽过氧化物酶(GSHPx)、S100A12、微小核糖核酸-130a(miR-130a)水平变化与继发性脑出血的相关性分析。方法:选取140例AIS患者,根据介入治疗后继发性脑出血发生情况分为发生组(n=60)与未发生组(n=80)。比较两组一般资料、治疗后血清GSHPx、S100A12、miR-130a水平,分析血清指标与继发性脑出血的相关性,多因素Logistic回归分析AIS患者介入治疗后发生继发性脑出血的危险因素,受试者工作特征(ROC)分析三者联合的预测价值。结果:发生组入院时NIHSS评分及治疗后血清GSHPx水平低于未发生组,治疗后血清S100A12、miR-130a水平高于未发生组(均P<0.05)。血清GSHPx水平与继发性脑出血的呈负相关(r=-0.602,P<0.05),血清S100A12、miR-130a水平与继发性脑出血呈正相关(r=0.510、r=0.423,均P<0.05),入院时NIHSS评分与治疗后GSHPx水平呈负相关,与治疗后S100A12、miR-130a水平呈正相关(均P<0.05)。三者均为AIS患者介入治疗后发生继发性脑出血的影响因素(P<0.05)。治疗后血清GSHPx、S100A12、miR-130a水平预测介入治疗后发生继发性脑出血的AUC分别为0.693、0.720、0.701,三者联合预测AUC为0.880,灵敏度、特异度均高于80.00%(均P<0.05)。结论:血清GSHPx、S100A12、miR-130a水平与AIS患者介入治疗后发生继发性脑出血有一定相关性,且三者联合对介入治疗后发生继发性脑出血有良好的预测价值。
Abstract:
Objective:To investigate the correlation between serum glutathione peroxidase (GSHPx),S100A12 and micrornA-130A (miR-130a) levels and secondary cerebral hemorrhage after acute ischemic stroke (AIS).Methods:A total of 140 AIS patients were selected and divided into two groups (n=60) and no group (n=80) according to the occurrence of secondary cerebral hemorrhage after interventional treatment.General data,serum GSHPx,S100A12 and miR-130a levels after treatment were compared between the two groups,and the correlation between serum indexes and secondary cerebral hemorrhage was analyzed.Multivariate Logistic regression analysis was conducted to analyze the risk factors for secondary cerebral hemorrhage in AIS patients after interventional therapy,and the predictive value of the combination of the three factors was analyzed by receiver operating characteristics (ROC) analysis.Results:The NIHSS score at admission and serum GSHPx level after treatment were lower in the occurrence group than in the non-occurrence group,and the serum S100A12 and miR-130a levels after treatment were higher than those in the non-occurrence group (all P<0.05).Serum GSHPx level was negatively correlated with secondary intracerebral hemorrhage (r=-0.602,P<0.05),and serum S100A12 and miR-130a levels were positively correlated with secondary intracerebral hemorrhage (r=0.510,r=0.423,all P<0.05).NIHSS score was negatively correlated with GSHPx level after treatment,and positively correlated with S100A12 and miR-130a level after treatment (all P<0.05).All the three factors were influencing the occurrence of secondary cerebral hemorrhage in AIS patients after interventional therapy (P<0.05).After treatment,serum GSHPx,S100A12 and miR-130a levels predicted the AUC of secondary intracerebral hemorrhage after interventional therapy was 0.693,0.720 and 0.701,respectively,and the AUC of the three combined predicted was 0.880,with sensitivity and specificity higher than 80.00% (P<0.05).Conclusion:The levels of serum GSHPx,S100A12 and miR-130a are correlated to the occurrence of secondary cerebral hemorrhage after interventional therapy in AIS patients,and the combination of the three has good predictive value for the occurrence of secondary cerebral hemorrhage after interventional therapy.
