摘要:
目的:探讨心外膜脂肪厚度(EFT)及血清半乳糖凝集素-3(Gal-3)、氧化三甲胺(TMAO)水平与心房颤动(AF)射频消融术后复发的相关性分析。方法:选取AF患者125例,根据AF射频消融术后复发情况分为复发组(n=25)和未复发组(n=100),比较两组EFT及血清Gal-3、TMAO水平,对AF射频消融术后复发的危险因素进行多因素Logistic分析。结果:截止2023年6月,所有患者均获得随访结果,无失访者,125例行AF射频消融术患者中,复发25例,未复发100例。两组年龄、性别、饮酒史、吸烟史、AF类型、手术时间、术中电复律、高血压、糖尿病患者占比之间比较无统计学差异(均P>0.05),与未复发组比较,复发组OSAS患者占比明显较高(均P<0.05)。与未复发组比较,复发组患者EFT、血清Gal-3、TMAO水平明显较高(均P<0.05)。多因素Logistic回归分析结果显示,AF射频消融术后复发的危险因素包括OSAS、高EFT、高血清Gal-3、TMAO水平(均P<0.05)。对AF射频消融术后复发的预测中EFT联合血清Gal-3、TMAO水平的AUC值(0.937)显著高于三者独立检测(均P<0.05)。结论:EFT及血清Gal-3、TMAO水平与AF射频消融术后复发存在一定相关性,且三者联合能够对术后复发情况进行较为准确的预测,有效识别AF复发高风险人群。
Abstract:
Objective:To investigate the correlation between epicardial adipose thickness (EFT),serum galectin-3 (Gal-3),trimethylamine oxide (TMAO) levels and recurrence of atrial fibrillation (AF) after radiofrequency ablation.Methods:A total of 125 patients with AF were selected and divided into recurrence group (n=25) and non-recurrence group (n=100) according to the recurrence of AF after radiofrequency ablation.The levels of EFT,serum Gal-3,and TMAO were contrasted between the two groups.Multivariate Logistic regression analysis was carried out to explore the risk factors contributing to AF recurrence after radiofrequency ablation.Results:Until June 2023,the follow-up of every patient was completed,and there was no loss to follow-up.Among 125 AF patients having radiofrequency ablation,25 patients relapsed and 100 patients remained free of recurrence.There was no significant difference in age,gender,operation time,intraoperative electrical cardioversion,hypertension,drinking history,smoking history,type of AF and diabetes between the two groups (all P>0.05).Compared with the non-recurrence group,the recurrence group had a significantly higher proportion of OSAS patients (all P<0.05).Compared with the non-recurrence group,the recurrence group had significantly higher levels of EFT,serum Gal-3,and TMAO (all P<0.05).According to the results of multivariate logistic regression analysis,OSAS,EFT,serum Gal-3 levels,and TMAO levels were identified as risk factors for the recurrence of AF following radiofrequency ablation(all P<0.05).For the prediction of AF recurrence after radiofrequency ablation using the combination of EFT,serum Gal-3,and TMAO levels,the area under the curve (AUC) was measured at 0.937,which was significantly higher than that of the three independent tests (all P<0.05).Conclusion:EFT,serum Gal-3,and TMAO levels show a specific correlation with AF recurrence post-radiofrequency ablation.The integration of these three factors enables precise prediction of postoperative recurrence in patients and effectively distinguishes the high-risk group for AF recurrence,making it highly suitable for clinical dissemination.
