摘要:
目的:探究血清谷胱甘肽过氧化物酶3(GPX3)、成纤维生长因子受体4(FGFR4)联合超声造影(CEUS)定量参数对甲状腺良恶性结节的鉴别诊断价值。方法:选取甲状腺结节患者150例,根据病理检查结果分为良性组(128例)和恶性组(22例)。比较两组血清GPX3、FGFR4水平以及CEUS特征和定量参数。Pearson法分析血清GPX3、FGFR4表达水平的相关性。受试者工作特征(ROC)曲线分析血清GPX3、FGFR4联合CEUS定量参数对甲状腺良恶性结节的鉴别诊断价值。结果:恶性组显影边界模糊、不均匀、显影时间晚、增强强度低、早消退比例高于良性组(均P<0.05)。与良性组比较,恶性组血清GPX3水平较低,FGFR4水平较高(均P<0.05)。恶性组平均渡越时间(MTT)和曲线下面积(AUCCEUS)低于良性组(均P<0.05)。ROC曲线分析显示,血清GPX3、FGFR4联合CEUS定量参数MTT、AUCCEUS对甲状腺良恶性结节的鉴别诊断价值优于单独检测(均P<0.05)。结论:血清GPX3、FGFR4联合CEUS定量参数MTT、AUCCEUS对甲状腺良恶性结节具有良好的鉴别诊断价值。
Abstract:
Objective:To explore the differential diagnostic value of serum glutathione peroxidase 3 (GPX3) and fibroblast growth factor receptor 4 (FGFR4) combined with contrast-enhanced ultrasound (CEUS) quantitative parameters for thyroid benign and malignant nodules.Methods:Patients with thyroid nodules (150 cases) were selected as the study subjects,and these patients were separated into benign group (128 cases) and malignant group (22 cases) based on the presence of malignant nodules.The serum GPX3,FGFR4 levels,and CEUS quantitative parameters were compared between the two groups.Pearson method was used to analyze the correlation between serum GPX3 and FGFR4 expression levels.In addition,ROC curve was used to analyze the differential diagnostic value of serum GPX3,FGFR4 combined with CEUS quantitative parameters for thyroid benign and malignant nodules.Results:The malignant group had higher proportions of patients with blurred and uneven development boundaries,late development,low enhancement intensity,and early fading than the benign group (all P<0.05).The malignant group had lower serum GPX3 level and higher FGFR4 level than the benign group (all P<0.05).The malignant group had lower MTT and AUCCEUS than the benign group (all P<0.05).ROC curve analysis showed that the value of serum GPX3 and FGFR4 combined with CEUS quantitative parameters MTT and AUCCEUS in the differential diagnosis of benign and malignant thyroid nodules was better than that of single detection (all P<0.05).Conclusion:Serum GPX3 and FGFR4 combined with CEUS quantitative parameters MTT and AUCCEUS have good diagnostic value for benign and malignant thyroid nodules.The efficacy of serum GPX3 and FGFR4 combined with CEUS quantitative parameters MTT and AUCCEUS in the adjunctive diagnosis of benign and malignant thyroid nodules is high,and it has certain clinical adjunctive differential diagnostic value.
