摘要:
目的:探讨2型糖尿病(T2DM)患者血清成纤维细胞生长因子21(FGF21)、解整合素样金属蛋白酶10(ADAM10)水平与糖尿病视网膜病变(DR)的关系。方法:选取T2DM患者90例为疾病组,根据是否发生DR分为DR组(37例)和NDR组(53例)。另选取同期体检健康者76例为对照组。采用ELISA法检测血清FGF21、ADAM10水平;多因素Logistic回归分析T2DM患者DR发生的影响因素;绘制受试者工作特征(ROC)曲线分析血清FGF21、ADAM10对T2DM患者DR的诊断价值。结果:疾病组血清FGF21、ADAM10水平高于对照组(均P<0.05)。DR组患者T2DM病程长于NDR组,合并高血压比例高于NDR组(均P<0.05)。与NDR组比较,DR组血清FGF21、ADAM10水平升高(均P<0.05)。血清FGF21、ADAM10是T2DM患者DR发生的影响因素(均P<0.05)。血清FGF21、ADAM10联合诊断的曲线下面积(AUC)高于两者单独诊断的AUC(均P<0.05)。结论:DR患者血清FGF21、ADAM10水平升高,两者联合检测对T2DM患者DR有较高的诊断价值。
Abstract:
Objective:To investigate the relationship between serum fibroblast growth factor 21 (FGF21),disintegrin and metalloproteinase 10 (ADAM10) levels and diabetic retinopathy (DR) in patients with type 2 diabetes mellitus (T2DM).Methods:A total of 90 patients with T2DM were selected as the disease group,which was further divided into the DR group (37 cases) and the non-DR (NDR) group (53 cases) based on the presence or absence of DR.In addition,76 healthy individuals who underwent physical examination during the same period were selected as the control group.The levels of serum FGF21 and ADAM10 were detected by ELISA.Multivariate logistic regression analysis was used to identify the influencing factors of DR in T2DM patients.ROC curves were plotted to analyze the diagnostic value of serum FGF21 and ADAM10 for DR in T2DM patients.Results:The levels of serum FGF21 and ADAM10 in the disease group were higher than those in the control group (all P<0.05).The duration of T2DM in the DR group was longer than that in the NDR group,and the proportion of patients with hypertension was higher in the DR group than in the NDR group (all P<0.05).Compared with the NDR group,the levels of serum FGF21 and ADAM10 were elevated in the DR group (all P<0.05).Serum FGF21 and ADAM10 were identified as influencing factors for the occurrence of DR in T2DM patients (all P<0.05).The AUC for the combined diagnosis of serum FGF21 and ADAM10 was higher than the AUC for either marker alone (all P<0.05).Conclusion:The levels of serum FGF21 and ADAM10 are elevated in patients with DR,and the combined detection of the two has a higher diagnostic value for DR in T2DM patients.
