摘要:
目的:探讨老年突发性耳聋患者血清高迁移率族蛋白B1(HMGB1)、缝隙连接蛋白43(Cx43)、血栓素B2(TXB2)水平变化及其检测意义。方法:选取老年突发性耳聋患者108例为突发性耳聋组,根据纯音听阈曲线将患者分为轻度耳聋组(42例)、中度耳聋组(36例)和重度耳聋组(30例)。另选93例无耳部疾病史的老年人为对照组。比较对照组和突发性耳聋组、不同听力损失程度患者血清HMGB1、Cx43、TXB2水平。采用多因素Logistic回归分析老年突发性耳聋发生的影响因素。结果:与对照组比较,突发性耳聋组血清HMGB1、TXB2水平升高,血清Cx43水平降低(均P<0.05)。轻度耳聋组患者血清HMGB1、TXB2水平低于中度耳聋组,且中度耳聋组低于重度耳聋组(均P<0.05)。轻度耳聋组患者血清Cx43水平高于中度耳聋组,且中度耳聋组高于重度耳聋组(均P<0.05)。与对照组比较,突发性耳聋组有糖尿病史、睡眠障碍、感冒病史、血清总胆固醇(TC)≥5.70 mmol/L及同型半胱氨酸(Hcy)≥16.00 μmol/L比例升高(均P<0.05)。有糖尿病史、睡眠障碍、感冒病史、血清TC≥5.70 mmol/L、Hcy≥16.00 μmol/L、HMGB1≥2.12 mg/L、TXB2≥209.89 pg/ml及Cx43≤147.34 pg/ml是老年突发性耳聋发生的危险因素(均P<0.05)。结论:老年突发性耳聋患者血清HMGB1、TXB2水平升高,血清Cx43水平降低,三者与听力损失程度有关,是老年突发性耳聋发生的影响因素。
Abstract:
Objective:To explore the changes in the levels of serum high mobility group protein B1 (HMGB1),connexin 43 (Cx43) and thromboxane B2 (TXB2) in elderly patients with sudden deafness and their detection significance.Methods:A total of 108 elderly patients with sudden deafness were selected as the sudden deafness group,and were further divided into mild hearing loss group (42 cases),moderate hearing loss group (36 cases) and severe hearing loss group (30 cases) based on pure tone audiometry threshold curve.Another 93 elderly individuals without a history of ear disease were chosen as the control group.The serum levels of HMGB1,Cx43 and TXB2 were compared between the control group and the sudden deafness group,as well as among patients with different degrees of hearing loss.Multivariate logistic regression analysis was used to identify the influencing factors for the occurrence of sudden deafness in the elderly.Results:Compared with the control group,the serum levels of HMGB1 and TXB2 were increased,while the serum level of Cx43 was decreased in the sudden deafness group (all P<0.05).The serum levels of HMGB1 and TXB2 in the mild hearing loss group were lower than those in the moderate hearing loss group,and the moderate hearing loss group had lower levels than the severe hearing loss group (all P<0.05).The serum level of Cx43 in the mild hearing loss group was higher than that in the moderate hearing loss group,and the moderate hearing loss group had a higher level than the severe hearing loss group (all P<0.05).Compared with the control group,the proportion of patients with a history of diabetes,sleep disorders,a history of cold,serum total cholesterol (TC)≥5.70 mmol/L and homocysteine (Hcy)≥16.00 μmol/L was increased in the sudden deafness group (all P<0.05).A history of diabetes,sleep disorders,a history of cold,serum TC≥5.70 mmol/L,Hcy≥16.00 μmol/L,HMGB1≥2.12 mg/L,TXB2≥209.89 pg/ml and Cx43≤147.34 pg/ml were identified as risk factors for sudden deafness in the elderly (all P<0.05).Conclusion:The serum levels of HMGB1 and TXB2 are increased,while the serum level of Cx43 is decreased in elderly patients with sudden deafness.These three factors are associated with the degree of hearing loss and are influencing factors for the occurrence of sudden deafness in the elderly.
