摘要:
目的:探讨慢性牙周炎(CP)患者龈沟液中程序性细胞死亡蛋白4(PDCD4)、Kruppel样因子6(KLF6)水平变化及其与病情严重程度的关系。方法:选取本院收治的155例CP患者为疾病组,另选取同期牙周检查健康者155例为对照组。根据分级标准将CP患者分为轻度组(62例)、中度组(50例)和重度组(43例)。采用ELISA法检测龈沟液PDCD4、KLF6水平。Pearson法分析CP患者龈沟液PDCD4、KLF6水平与牙周指标、牙槽骨吸收程度及龈沟液炎症因子水平的相关性。采用多因素Logistic回归分析CP患者病情严重程度的影响因素。绘制受试者工作特征(ROC)曲线分析龈沟液PDCD4、KLF6对重度CP的诊断价值。结果:与对照组比较,疾病组牙龈指数(GI)、探诊深度(PD)、附着丧失(AL)、出血指数(SBI)、菌斑指数(PLI)及牙槽骨吸收程度升高(均P<0.05)。与对照组比较,疾病组龈沟液白细胞介素(IL)-1β、IL-6、肿瘤坏死因子-α(TNF-䥺SymbolaA@)、KLF6水平升高,PDCD4水平降低(均P<0.05)。CP患者龈沟液PDCD4水平与GI、PD、AL、SBI、PLI、牙槽骨吸收程度及龈沟液IL-1β、IL-6、TNF-α水平呈负相关(均P<0.05)。龈沟液KLF6水平与GI、PD、AL、SBI、PLI、牙槽骨吸收程度及龈沟液IL-1β、IL-6、TNF-α水平呈正相关(均P<0.05)。轻度组、中度组和重度组患者吸烟比例、GI、SBI、PLI、龈沟液IL-1β、IL-6、TNF-α、KLF6水平依次升高,龈沟液PDCD4水平依次降低(均P<0.05)。龈沟液PDCD4、KLF6是重度CP的独立影响因素(均P<0.05)。龈沟液PDCD4、KLF6联合诊断的曲线下面积(AUC)高于两者单独诊断的AUC(均P<0.05)。结论:CP患者龈沟液中PDCD4表达下调,KLF6表达上调,与病情严重程度有关,是重度CP的独立影响因素,两者联合检测对重度CP具有一定诊断价值。
Abstract:
Objective:To investigate the changes of programmed cell death protein 4 (PDCD4) and Kruppel-like factor 6 (KLF6) levels in gingival crevicular fluid of patients with chronic periodontitis (CP) and their relationship with the severity of the disease.Methods:A total of 155 patients with CP treated in our hospital were selected as the disease group,and another 155 individuals with healthy periodontal status during the same period were selected as the control group.According to the grading criteria,patients with CP were divided into mild group (62 cases),moderate group (50 cases) and severe group (43 cases).The levels of PDCD4 and KLF6 in gingival crevicular fluid were measured using the ELISA method.Pearson correlation analysis was used to assess the correlation between gingival crevicular fluid levels of PDCD4 and KLF6 and periodontal indices,alveolar bone resorption degree,and inflammatory cytokines in gingival crevicular fluid.Multivariate logistic regression analysis was performed to identify the factors influencing the severity of CP.ROC curves were plotted to evaluate the diagnostic value of gingival crevicular fluid PDCD4 and KLF6 for severe CP.Results:Compared with the control group,the disease group had significantly higher Gingival Index (GI),probing depth (PD),attachment loss (AL),Bleeding Index (SBI),Plaque Index (PLI) and alveolar bone resorption degree (all P<0.05).Compared with the control group,the disease group had elevated levels of interleukin (IL)-1β,IL-6,tumor necrosis factor-α (TNF-α),and KLF6 in gingival crevicular fluid,while PDCD4 levels were reduced (all P<0.05).In CP patients,the level of PDCD4 in gingival crevicular fluid was negatively correlated with GI,PD,AL,SBI,PLI,alveolar bone resorption degree,and gingival crevicular fluid levels of IL-1β,IL-6 and TNF-α (all P<0.05).The level of KLF6 in gingival crevicular fluid was positively correlated with GI,PD,AL,SBI,PLI,alveolar bone resorption degree,and gingival crevicular fluid levels of IL-1β,IL-6 and TNF-α (all P<0.05).The proportion of smokers,GI,SBI,PLI,and gingival crevicular fluid levels of IL-1β,IL-6,TNF-α and KLF6 increased progressively from the mild to severe groups,while the gingival crevicular fluid level of PDCD4 decreased progressively (all P<0.05).Gingival crevicular fluid PDCD4 and KLF6 were independent influencing factors for severe CP (all P<0.05).The AUC for combined diagnosis of gingival crevicular fluid PDCD4 and KLF6 was higher than that for individual diagnosis (all P<0.05).Conclusion:In CP patients,downregulation of gingival crevicular fluid PDCD4 and upregulation of KLF6 are associated with disease severity and are independent influencing factors for severe CP.Combined detection of these two factors has diagnostic value for severe CP.
