摘要:
目的:探讨血清单核细胞趋化蛋白-4(MCP-4)、心脏型脂肪酸结合蛋白(FABP3)、趋化因子配体9(CXCL9)水平与重症支原体肺炎(SMPP)心肌损害的相关性。方法:选取重症支原体肺炎合并心肌损害患者158例为SMPP组,按照是否发生心肌损害分为心肌损害组(n=42)和非心肌损害组(n=116),另选102例同期健康体检者为对照组。比较两组血清MCP-4、FABP3、CXCL9水平,记录患者临床一般资料,多因素Logistic分析重症支原体患者合并肺炎心肌损害的危险因素。结果:对照组比较,SMPP组患者的血清MCP-4、FABP3、CXCL9水平明显升高(均P<0.05),与非心肌损害组比较,心肌损害组患者血清MCP-4、FABP3、CXCL9水平明显升高(均P<0.05)。两组年龄、性别、慢性阻塞性肺病(COPD)、糖尿病、吸烟史、黄疸、低氧血症情况比较均无统计学差异(均P>0.05),与非心肌损害组比较,心肌损害组患者高血压、冠心病、贫血患者占比、血清MCP-4、FABP3、CXCL9水平明显升高(均P<0.05)。合并高血压、冠心病、贫血、高血清MCP-4、FABP3、CXCL9水平是重症支原体患者并发心肌损害的危险因素。患者血清MCP-4、FABP3、CXCL9水平与重症支原体患者并发心肌损害发生率呈正相关(均P<0.05)。结论:血清MCP-4、FABP3、CXCL9水平与重症支原体肺炎患者合并心肌损害呈正相关,且合并高血压、冠心病、贫血、高血清MCP-4、FABP3、CXCL9水平是重症支原体患者并发心肌损害的危险因素,三者可作为临床重症支原体患者并发心肌损害的生物学指标使用,对患者病情评估及治疗方案的制定具有重要意义。
Abstract:
Objective:To investigate the correlation between serum monocyte chemotactic protein-4(MCP-4),cardiotype fatty acid-binding protein(FABP3),chemokine ligand 9(CXCL9) levels and myocardial damage in severe mycoplasma pneumonia(SMPP).Methods:A total of 158 patients with severe mycoplasma pneumonia complicated with myocardial damage were selected as SMPP group,and divided into myocardial damage group(n=42) and non-myocardial damage group(n=116) according to whether myocardial damage occurred,and 102 healthy subjects were selected as control group.The serum levels of MCP-4,FABP3 and CXCL9 were compared between the two groups,and the general clinical data of the patients were recorded.The risk factors of myocardial damage in severe mycoplasma patients complicated with pneumonia were analyzed by multivariate Logistic analysis.Results:Compared with the control group,the serum levels of MCP-4,FABP3 and CXCL9 in SMPP group were significantly increased(all P<0.05),and the serum levels of MCP-4,FABP3 and CXCL9 in myocardial damage group were significantly increased compared with the non-myocardial damage group(all P<0.05).There were no significant differences in age,sex,chronic obstructive pulmonary disease(COPD),diabetes,smoking history,jaundice and hypoxemia between the two groups(all P>0.05).Compared with the non-myocardial damage group,the proportion of patients with hypertension,coronary heart disease and anemia,and the serum levels of MCP-4,FABP3 and CXCL9 in the myocardial damage group were significantly increased(all P<0.05).Hypertension,coronary heart disease,anemia,high serum levels of MCP-4,FABP3,CXCL9 are risk factors for myocardial damage in patients with severe mycoplasma.Serum levels of MCP-4,FABP3 and CXCL9 were positively correlated with the possibility of myocardial damage in mycoplasma patients(P<0.05).Conclusion:Serum MCP-4,FABP3,CXCL9 levels were positively correlated with the possibility of myocardial damage in patients with severe mycoplasma pneumonia,and hypertension,coronary heart disease,anemia,high serum MCP-4,FABP3,CXCL9 levels were risk factors for myocardial damage in patients with severe mycoplasma pneumonia,the three can be used as biological indicators of myocardial damage in patients with severe mycoplasma,which is of great significance for the assessment of patients’ condition and the formulation of treatment plan.
