摘要:
目的:探讨S-腺苷蛋氨酸结构域蛋白2(RSAD2)mRNA在系统性红斑狼疮(SLE)患者外周血单个核细胞(PBMCs)中的表达及其对贝利尤单抗治疗应答的预测价值。方法:将接受贝利尤单抗治疗的48例SLE患者纳入SLE组,将年龄、性别匹配的健康女性48例纳入健康对照组,比较两组PBMCs中RSAD2 mRNA表达水平,以及治疗前后SLE组生化指标[白细胞计数(WBC)、血小板计数(PLT)、C-反应蛋白(CRP)、血沉(ESR)、白蛋白(ALB)、24 h尿蛋白(24 hUP)]、免疫学指标[抗dsDNA抗体、IgG、补体C3、补体C4]和系统性红斑狼疮疾病活动指数2000(SLEDAI-2K)评分,分析治疗前RSAD2 mRNA表达水平对预测贝利尤单抗治疗应答的价值。结果:与治疗前比较,SLE患者在接受贝利尤单抗治疗3、6、12个月后生化指标(除WBC和PLT外)、免疫学指标及SLEDAI-2K评分等明显好转(均P<0.05)。SLE组治疗前RSAD2 mRNA表达较健康对照组明显升高,且其表达水平与SLEDAI-2K评分呈正相关(均P<0.05)。SLE组接受贝利尤单抗治疗12个月的狼疮反应指数4(SRI-4)应答率为75.00%,且应答患者治疗前RSAD2 mRNA表达高于未应答患者(P<0.01)。治疗前RSAD2 mRNA表达对贝利尤治疗第12个月时的应答具有一定的预测价值(P<0.01),敏感度为61.11%,特异度为83.33%。结论:RSAD2 mRNA在SLE患者外周血PBMCs中高表达,对SLE患者贝利尤单抗治疗应答具有良好的预测价值。
Abstract:
Objective:To investigate the expression of radical S-adenosyl methionine domain containing 2 (RSAD2) mRNA in peripheral blood mononuclear cells (PBMCs) of systemic lupus erythematosus (SLE) patients and its predictive value for the response to belimumab treatment.Methods:A total of 48 SLE patients treated with belimumab were included in the SLE group,and 48 healthy women matched for age and sex were included in the healthy control group.The expression level of RSAD2 mRNA in PBMCs between the two groups,and biochemical indicators (WBC,PLT,CRP,ESR,ALB,24 hUP) and immunological indexes [anti-dsDNA antibody,IgG,complement C3,complement C4] and SLEDAI-2K score in SLE group before and after treatment were compared.The value of RSAD2 mRNA expression level before treatment in predicting the treatment response of belimumab was analyzed.Results:Biochemical parameters (except WBC and PLT),immunological parameters and SLEDAI-2K scores were significantly improved after 3,6 and 12 months of belimumab treatment compared with pre-treatment (all P<0.05).The expression of RSAD2 mRNA in the SLE group was significantly higher than that in the healthy control group before treatment,and its expression level was positively correlated with SLEDAI-2K score (all P<0.05).The response rate of SRI-4 in SLE patients treated with belimumab for 12 months was 75.00%,and the expression of RSAD2 mRNA in responders was higher than that in non-responders before treatment (P<0.01).RSAD2 mRNA expression before treatment had a certain predictive value for the response to belimumab at month 12 (P<0.01),with a sensitivity of 61.11% and a specificity of 83.33%.Conclusion:RSAD2 mRNA is highly expressed in peripheral blood PBMCs of SLE patients and has a good predictive value for the response to belimumab in SLE patients.
