摘要:
目的:评估彩色多普勒超声联合血清三叶因子1(TFF1)、四次跨膜蛋白1(TM4SF1)对甲状腺结节(TN)良恶性的鉴别诊断价值。方法:选取TN患者143例为研究对象,以术后的病理诊断结果为金标准分为TN良性组(102例)及TN恶性组(41例)。采用飞利浦EPIO7彩色多普勒超声诊断仪扫描患者颈部获得超声图像;采用酶联免疫法检测血清TFF1、TM4SF1水平;采用受试者工作特性(ROC)曲线评估血清TFF1、TM4SF1对TN良恶性的鉴别诊断价值;采用Kappa检验和四格表法分别分析彩色多普勒超声、血清TFF1、血清TM4SF1水平及三者联合对TN性质的诊断结果与病理诊断结果的一致性及诊断价值。结果:两组患者彩色多普勒超声图像特征中,淋巴结是否肿大、形态是否规则、边界是否清晰、钙化情况、内部回声、血流分级比较差异有统计学意义(均P<0.05)。TN恶性组血清TFF1、TM4SF1水平高于TN良性组(均P<0.05)。血清TFF1、TM4SF1鉴别诊断TN良恶性的曲线下面积(AUC)分别为0.836、0.848,截断值分别为1.19、133.95 ng/ml。彩色多普勒超声联合血清TFF1、TM4SF1鉴别诊断TN良恶性的结果与病理诊断结果呈高度一致性(κ=0.757,P=0.000)。彩色多普勒超声联合血清TFF1、TM4SF1鉴别诊断TN良恶性的特异度高于三者单独检测,阳性预测率及准确率高于血清TFF1、TM4SF1单独检测(均P<0.05)。结论:TN良性与恶性患者彩色多普勒超声征象及血清TFF1、TM4SF1水平均有明显差异,三者对于TN良恶性的鉴别诊断均有一定的临床价值,三者联合可提高鉴别诊断价值。
Abstract:
Objective:To evaluate the value of color Doppler ultrasound combined with serum trefoil factor 1 (TFF1) and Transmembrane 4 L six family member 1 (TM4SF1) in the differential diagnosis of benign and malignant thyroid nodules (TN).Methods:A total of 143 TN patients were selected as the research subjects.The postoperative pathological diagnosis results were used as the gold standard,and the patients were assigned into TN benign group (102 cases) and TN malignant group (41 cases).The Philips EPIO7 color Doppler ultrasound diagnostic device was used to scan neck to obtain ultrasound images.Enzyme linked immunosorbent assay was used to detect serum TFF1 and TM4SF1.ROC curve was used to evaluate the value of serum TFF1 and TM4SF1 in the differential diagnosis of benign and malignant TN.Kappa test and four grid table method were used to analyze the consistency of color Doppler ultrasound,serum TFF1,serum TM4SF1,and their combination in the diagnosis of TN properties and pathological diagnosis results,respectively,and their diagnostic value.Results:There were obvious differences between TN malignant group and TN benign group in terms of lymph node enlargement,TN morphology regularity,clear boundaries,calcification,internal echo,and blood flow grading (all P<0.05).The TN malignant group had obviously higher serum TFF1 and TM4SF1 than TN benign group (all P<0.05).The AUC values for distinguishing between benign and malignant TN using serum TFF1 and TM4SF1 were 0.836 and 0.848,respectively,with cut-off values of 1.19 and 133.95 ng/ml,respectively.The results of color Doppler ultrasound combined with serum TFF1 and TM4SF1 in distinguishing benign and malignant TN were highly consistent with pathological diagnosis (κ=0.757,P=0.000).The specificity of color Doppler ultrasound combined with serum TFF1 and TM4SF1 in the differential diagnosis of benign and malignant TN was higher than that of the three alone,and the positive prediction rate and accuracy were higher than those of serum TFF1 and TM4SF1 alone (all P<0.05).Conclusion:There are obvious differences in color Doppler ultrasound signs and serum TFF1 and TM4SF1 levels between TN benign and TN malignant patients.The three have certain clinical value for the differential diagnosis of TN benign and malignant,and their combination can improve the diagnostic value.
