摘要:
目的:检测急性胰腺炎(AP)患者血清活化素A(ACT-A)、乳酸脱氢酶与白蛋白比值(LAR)、活化基质金属蛋白酶-8(aMMP-8)水平,分析其临床意义。方法:选取AP患者80例,根据胰腺影像学表现分为水肿型AP组(60例)和坏死型AP组(20例); 根据AP严重程度分为轻症AP组(49例)和重症AP组(31例); 根据预后状况分为预后良好组(50例)和预后不良组(30例)。检测血清ACT-A、LAR、aMMP-8表达水平并进行组间比较。分析影响AP患者预后的因素,以及血清ACT-A、LAR、aMMP-8对AP患者预后的预测价值。结果:坏死型AP组较水肿型AP组血清ACT-A、LAR、aMMP-8水平升高(均P<0.05)。重症AP组较轻症AP组血清ACT-A、LAR、aMMP-8水平升高(均P<0.05)。预后不良组较预后良好组入院时APACHE Ⅱ评分以及血清ACT-A、LAR、aMMP-8水平升高(均P<0.05)。ACT-A、LAR、aMMP-8是AP患者发生预后不良的危险因素(均P<0.05)。血清ACT-A、LAR、aMMP-8联合预测AP患者发生预后不良的价值优于单项(均P<0.05)。结论:AP患者血清ACT-A、LAR、aMMP-8水平均升高,与患者病情严重程度有关,联合检测对患者预后具有良好的预测价值。
Abstract:
Objective:To detect serum activin A(ACT-A),lactate dehydrogenase to albumin ratio(LAR)and activated matrix metalloproteinase-8(aMMP-8)levels in patients with acute pancreatitis(AP),and to analyze their clinical significance.Methods:A total of 80 AP patients were selected and divided into edematous AP group(60 cases)and necrotic AP group(20 cases)according to the pancreatic imaging findings; according to the severity of AP,the patients were divided into mild AP group(49 cases)and severe AP group(31 cases); according to the prognosis,the patients were divided into good prognosis group(50 cases)and poor prognosis group(30 cases).The expression levels of ACT-A,LAR and aMMP-8 in serum were detected and compared between groups.Logistic regression analysis was used to The factors affecting the prognosis and the predictive value of serum ACT-A,LAR and aMMP-8 for the prognosis of AP patients were analyzed.Results:Compared with the edematous AP group,the necrotic AP group had significantly higher serum ACT-A,LAR,and aMMP-8 levels(all P<0.05).The serum levels of ACT-A,LAR and aMMP-8 in severe AP group were higher than those in mild AP group(all P<0.05).The poor prognosis group had higher APACHE Ⅱ score and serum ACT-A,LAR and aMMP-8 levels compared to good prognosis group(all P<0.05).ACT-A,LAR and aMMP-8 were risk factors for poor prognosis in AP patients(all P<0.05).The combination of ACT-A,LAR and aMMP-8 was better than any single item in predicting poor prognosis in AP patients(all P<0.05).Conclusion:The serum levels of ACT-A,LAR and aMMP-8 are increased in AP patients,which are related to the severity of the disease.The combined detection has a good predictive value for the prognosis of AP patients.
