摘要:
目的:检测川崎病(KD)患儿血清血管生成素样蛋白8(ANGPTL8)、生长分化因子-15(GDF-15)水平,并分析其与并发冠状动脉病变(CAL)的关系。方法:选取136例KD患儿为研究对象,依据是否并发CAL分为CAL组(69例)和非CAL组(67例)。选择同期70例健康体检患儿为对照组。采用酶联免疫吸附法(ELISA)检测血清ANGPTL8、GDF-15水平。比较三组一般资料及血清ANGPTL8、GDF-15水平,以及不同CAL病情严重程度KD患儿血清ANGPTL8、GDF-15水平。分析KD患儿并发CAL的影响因素,以及血清ANGPTL8、GDF-15水平对KD患儿并发CAL的预测价值。结果:CAL组和非CAL组血小板计数(PLT)、白细胞计数(WBC)、C-反应蛋白(CRP)、红细胞沉降率(ESR)、ANGPTL8、GDF-15水平高于对照组,血红蛋白(Hb)、白蛋白(ALB)水平低于对照组(均P<0.05)。CAL组CRP、ESR、ANGPTL8、GDF-15水平高于非CAL组,Hb水平低于非CAL组(均P<0.05)。随着患儿CAL病情加重,血清ANGPTL8、GDF-15水平逐步升高(均P<0.05)。CRP、ESR、ANGPTL8、GDF-15水平是KD患儿并发CAL的影响因素(均P<0.05)。血清ANGPTL8、GDF-15水平联合预测并发CAL的价值高于单项(均P<0.05)。结论:KD患儿血清ANGPTL8、GDF-15水平升高,与CAL病情严重程度相关,两者联合预测并发CAL的价值较高。
Abstract:
Objective:To detect the serum levels of angiopoietin-like protein 8(ANGPTL8)and growth differentiation factor-15(GDF-15)in children with Kawasaki disease(KD),and to analyze their relationship with concurrent coronary artery lesions(CAL).Methods:A total of 136 children were selected and divided into the CAL group(69 cases)and the non-CAL group(67 cases)according to whether they had CAL or not,and 70 children who underwent physical examination were selected as the control group at the same time.ELISA was applied to detect serum levels of ANGPTL8 and GDF-15.The general information and serum levels of ANGPTL8 and GDF-15 were compared among children in three groups,and the serum levels of ANGPTL8 and GDF-15 in KD children with different severity levels of CAL were compared.The influencing factors of concurrent CAL in children with KD,and the predictive value of serum ANGPTL8 and GDF-15 levels for concurrent CAL in children with KD were analyzed.Results:Platelet count(PLT),white blood cell count(WBC),C-reactive protein(CRP),erythrocyte sedimentation rate(ESR),ANGPTL8,and GDF-15 levels in the CAL group and non-CAL group were higher than those in the control group,while hemoglobin(Hb)and albumin(ALB)levels were lower than those in the control group(all P<0.05).The CRP,ESR,ANGPTL8,and GDF-15 levels in the CAL group were higher than those in the non-CAL group,while the Hb level was lower than that in the non-CAL group(all P<0.05).The serum levels of ANGPTL8 and GDF-15 gradually increased with the severity of CAL(all P<0.05).CRP,ESR,ANGPTL8,and GDF-15 levels were influencing factors for concurrent CAL in KD children(all P<0.05).The combined value of serum ANGPTL8 and GDF-15 levels in predicting concurrent CAL was higher than that of each item alone(all P<0.05).Conclusion:The levels of serum ANGPTL8 and GDF-15 in KD children are elevated,and they are correlated with the severity of CAL.The combination of both has good value for predicting the occurrence CAL.
