摘要:
目的:探讨血管内皮生长因子(VEGF)、程序性死亡受体-1(PD-1)联合胃功能三项对早期胃癌(EGC)的诊断及预后评估价值。方法:选取EGC患者167例为EGC组,对术中的胃癌及癌旁组织进行收集,免疫组织化学染色后比较VEGF、PD-1阳性表达情况。另选取本院同期体检健康者74例为对照组,比较EGC组与对照组胃功能三项[胃蛋白原Ⅰ(PG Ⅰ)、胃蛋白原Ⅱ(PG Ⅱ)和胃泌素17(G17)]水平。所有患者均进行为期3年随访,根据是否病死分为预后良好组和预后不良组,比较两组临床病理资料。Logistic回归分析EGC患者预后不良的影响因素,Pearson法分析各指标的相关性,受试者工作特征(ROC)曲线分析VEGF、PD-1联合胃功能三项检测在EGC患者诊断及预后评估中的应用价值。结果:EGC组患者癌旁组织VEGF、PD-1阳性表达率低于胃癌组织(均P<0.05)。与对照组比较,EGC组PG Ⅰ水平降低,PG Ⅱ、G17水平升高(均P<0.05)。根据随访结果,预后不良组59例,预后良好组108例。预后不良组患者VEGF和PD-1阳性表达率以及PGⅡ、G17水平高于预后良好组,中高分化占比和PG Ⅰ水平低于预后良好组(均P<0.05)。低PG Ⅰ水平、高VEGF和PD-1阳性表达率、低分化、高PG Ⅱ和G17水平是EGC患者预后不良的危险因素(均P<0.05)。VEGF、PD-1、PG Ⅱ、G17水平与分化程度呈负相关,PG Ⅰ水平与分化程度呈正相关(均P<0.05)。VEGF、PD-1联合胃功能三项检测对EGC患者均有一定的诊断和预后评估价值,联合检测的价值更高(均P<0.05)。结论:VEGF、PD-1在胃癌组织中高表达,两者联合胃功能三项对EGC的诊断和预后评估均有较高价值。
Abstract:
Objective:To investigate the value of vascular endothelial growth factor(VEGF),programmed death receptor-1(PD-1)combined with gastric function in diagnosis and prognosis evaluation of early gastric cancer(EGC).Methods:A total of 167 patients with EGC were selected as the EGC group.The intraoperative gastric cancer tissues and adjacent tissues were collected,and the positive expressions of VEGF and PD-1 were compared after immunohistochemical staining.Another 74 healthy subjects were selected as the control group.The levels of gastric function indexes(PGⅠ,PGⅡ and G17)were compared between the EGC group and the control group.All patients were followed up for 3 years,and were divided into good prognosis group and poor prognosis group according to whether they died.The clinicopathological data of the two groups were compared.Logistic regression was used to analyze the influencing factors of poor prognosis in EGC patients.Pearson method was used to analyze the correlation of each index.ROC curve was used to analyze the application value of VEGF,PD-1 combined with gastric function in the diagnosis and prognosis evaluation of EGC patients.Results:The positive expression rates of VEGF and PD-1 in adjacent tissues of EGC group were lower than those in gastric cancer tissues(both P<0.05).Compared with the control group,the level of PG Ⅰ was decreased,and the levels of PG Ⅱ and G17 were increased in the EGC group(all P<0.05).According to the follow-up results,59 patients were in the poor prognosis group and 108 patients were in the good prognosis group.The positive expression rates of VEGF and PD-1,and the levels of PG Ⅱ and G17 in the poor prognosis group were higher than those in the good prognosis group,and the proportion of moderately and well differentiated and the level of PG Ⅰ in the poor prognosis group were lower than those in the good prognosis group(all P<0.05).Low PG Ⅰ level,high positive expression rates of VEGF and PD-1,poor differentiation,and high PG Ⅱ and G17 levels were risk factors for poor prognosis of EGC patients(all P<0.05).The levels of VEGF,PD-1,PG Ⅱ and G17 were negatively correlated with the degree of differentiation,and the level of PGⅠ was positively correlated with the degree of differentiation(all P<0.05).The combined detection of VEGF,PD-1 and gastric function had certain diagnostic and prognostic value for EGC patients,and the combined detection had higher value(all P<0.05).Conclusion:VEGF and PD-1 are both highly expressed in gastric cancer tissues,and the combination of VEGF,PD-1 and gastric function has high value in the diagnosis and prognosis evaluation of EGC.
