摘要:
目的:探讨英国前瞻性糖尿病研究(UKPDS)评分联合下肢动脉病变分级对2型糖尿病(T2DM)患者冠状动脉病变(CAD)程度的预测价值。方法:选择行冠状动脉造影的T2DM患者213例,依据冠状动脉造影结果分为T2DM组(75例)和T2DM合并CAD组(138例)。收集患者临床资料,检测患者糖脂代谢指标,使用UKPDS评分评估10年冠心病风险(CADRISK)。以冠状动脉SYNTAX评分对患者CAD程度进行评估,并分为高分组(≥23分)和低分组(<23分),比较两组CADRISK值及下肢动脉病变分级。分析T2DM患者CAD影响因素。分析CADRISK值、下肢动脉病变分级与SYNTAX评分的相关性,以及两者对T2DM合并CAD的预测价值。结果:T2DM合并CAD组和T2DM组在年龄、病程、收缩压、总胆固醇(TC)、下肢动脉病变分级、CADRISK值、SYNTAX评分上比较,差异有统计学意义(均P<0.05)。病程、TC、下肢动脉病变分级、CADRISK值、SYNTAX评分是T2DM合并CAD的影响因素(均P<0.05)。依据SYNTAX评分将患者分为高分组(26例)和低分组(56例),低分组CADRISK值低于高分组,高分组下肢动脉病变分级2~3级占比高于低分组(均P<0.05)。病程、TC、下肢动脉病变分级、CADRISK值与SYNTAX评分呈正相关(均P<0.05)。CADRISK值、下肢动脉病变分级均可预测T2DM患者CAD程度,两项联合预测价值更高(均P<0.05)。结论:UKPDS评分、下肢动脉病变分级与T2DM患者CAD程度相关,两者联合对T2DM合并CAD有较好的预测价值。
Abstract:
Objective:To investigate the predictive value of the UK Prospective Diabetes Study(UKPDS)score combined with lower extremity arterial disease classification in the degree of coronary artery disease in patients with type 2 diabetes mellitus(T2DM).Methods:A total of 213 T2DM patients who underwent coronary angiography were divided into T2DM group(75 cases)and T2DM with CAD group(138 cases)according to the results of coronary angiography.The 10-year coronary heart disease risk(CADRISK)was assessed by UKPDS.The degree of CAD was evaluated according to the SYNTAX score of coronary artery,and the patients were divided into high score group(SYNTAX score ≥23)and low score group(SYNTAX score <23).CADRISK value and lower extremity arterial disease classification were compared between the two groups.The influencing factors of in T2DM with CAD patients were analyzed.The correlation between CADRISK value,lower extremity arterial disease classification and SYNTAX score was analyzed,and the predictive value of both of them for T2DM with CAD was analyzed.Results:There were significant differences in age,disease duration,systolic blood pressure,total cholesterol,lower extremity arterial disease grading,CADRISK score,and SYNTAX score between the T2DM with CAD group and the T2DM group(all P<0.05).The course of disease,TC,lower extremity arterial disease classification,CADRISK score and SYNTAX score were the influencing factors of CAD in T2DM(all P<0.05).Patients were divided into high score group(26 cases)and low score group(56 cases)according to SYNTAX score,and CADRISK value in low score group was lower than that in high score group,and the proportion of lower extremity arterial disease grade 2 to 3 in high score group was higher than that in low score group(all P<0.05).The course of disease,TC,lower extremity arterial disease classification,CADRISK value were positively correlated with SYNTAX score(all P<0.05).Both CADRISK value and lower extremity arterial disease classification could predict the degree of CAD in T2DM patients,and the combination of the two had a higher predictive value(all P<0.05).Conclusion:The UKPDS score and lower extremity arterial disease classification are associated with the severity of CAD in T2DM patients,and the combination of the two has a good predictive value for CAD in T2DM patients.
