摘要:
目的:探讨可溶性转铁蛋白受体(sTfR)、膜铁转运蛋白1(FPN1)、对氧磷酶1(PON1)在缺铁性贫血(IDA)孕妇中的表达及意义。方法:选取IDA孕妇182例为IDA组,根据IDA严重程度将IDA组分为轻度组(80例)、中度组(61例)和重度组(41例),另选取同期健康孕妇73例为健康对照组。比较各组孕妇血清sTfR、FPN1、PON1水平。比较IDA组和健康对照组贫血相关指标。分析血清sTfR、FPN1、PON1水平与血红蛋白(Hb)、平均红细胞体积(MCV)、血清铁蛋白(SF)水平的相关性。绘制受试者工作特征(ROC)曲线分析血清sTfR、FPN1、PON1对孕妇IDA的诊断价值。记录所有孕妇不良结局发生情况,根据最终母婴结局将所有研究对象分为母婴结局正常组(179例)和母婴结局不良组(76例)。比较不同母婴结局孕妇血清sTfR、FPN1、PON1水平。结果:与健康对照组比较,IDA组sTfR、FPN1水平升高,PON1水平降低(均P<0.05)。IDA组Hb、MCV、SF水平低于健康对照组(均P<0.05)。血清sTfR、FPN1与Hb、MCV、SF呈负相关,PON1与Hb、MCV、SF呈正相关(均P<0.05)。血清sTfR、FPN1、PON1对孕妇IDA均有一定诊断效能,且联合检测诊断价值更高(均P<0.05)。重度组血清sTfR、FPN1水平高于轻度和中度组,PON1水平低于轻度和中度组(均P<0.05)。与健康对照组比较,IDA组母婴不良结局总发生率更高(P<0.05)。与母婴结局正常组比较,母婴结局不良组血清sTfR、FPN1水平升高,PON1水平降低(均P<0.05)。结论:血清sTfR、FPN1、PON1在IDA孕妇中均呈异常表达,三者联合检测对IDA诊断效能较高,且与IDA严重程度和母婴结局有关。
Abstract:
Objective:To investigate the expression and significance of soluble transferrin receptor(sTfR),ferroportin 1(FPN1)and paraoxonase 1(PON1)in pregnant women with iron deficiency anemia(IDA).Methods:A total of 182 pregnant women with IDA were selected as IDA group,and the IDA group was divided into mild group(80 cases),moderate group(61 cases)and severe group(41 cases)according to the severity of IDA,and 73 healthy pregnant women were selected as healthy control group.The serum levels of sTfR,FPN1 and PON1 were compared among all groups.The anemia-related indexes of IDA group and healthy control group were compared.The correlation between serum sTfR,FPN1,PON1 levels and hemoglobin(Hb),mean corpuscular volume(MCV)and serum ferritin(SF)levels was analyzed.Receiver operating characteristic(ROC)curve was drawn to analyze the diagnostic value of serum sTfR,FPN1 and PON1 for IDA in pregnant women.Adverse outcomes of all pregnant women were recorded,and all subjects were divided into normal maternal and infant outcome group(179 cases)and adverse maternal and infant outcome group(76 cases)according to the final maternal and infant outcome.The serum sTfR,FPN1 and PON1 levels of pregnant women with different outcomes were compared.Results:Compared with the healthy control group,the levels of sTfR and FPN1 in IDA group were increased,and the levels of PON1 were decreased(all P<0.05).The levels of Hb,MCV and SF in IDA group were lower than those in healthy control group(all P<0.05).Serum sTfR and FPN1 were negatively correlated with Hb,MCV and SF,while PON1 was positively correlated with Hb,MCV and SF(all P<0.05).Serum sTfR,FPN1 and PON1 had certain diagnostic efficacy for IDA in pregnant women,and the combined detection had higher diagnostic value(all P<0.05).Serum sTfR and FPN1 levels in severe group were higher than those in mild and moderate groups,and PON1 levels were lower than those in mild and moderate groups(all P<0.05).Compared with the healthy control group,the overall incidence of adverse outcomes was higher in IDA group(P<0.05).Compared with the normal maternal and infant outcome group,the serum sTfR and FPN1 levels were increased and PON1 levels were decreased in the poor maternal and infant outcome group(all P<0.05).Conclusion:Serum sTfR,FPN1 and PON1 are abnormally expressed in pregnant women with IDA,and the combined detection of the three has high diagnostic efficacy in the diagnosis of IDA,and they are related to the severity of IDA and maternal and infant outcomes.
