摘要:
目的:检测糖尿病酮症酸中毒合并感染患者血清微小RNA(miR)-146a和miR-155水平,分析其意义。方法:选取DKA合并感染患者76例(感染组)和单纯DKA未合并感染患者60例(未感染组)。入院后24 h内,对实验室生化指标进行检测,采用实时荧光定量聚合酶链反应(RT-qPCR)测定血清miR-146a、miR-155水平。分析DKA合并感染的影响因素, DKA患者血清miR-146a、miR-155、白细胞计数(WBC)和C-反应蛋白(CRP)的相关性,以及血清miR-146a、miR-155对DKA合并感染的诊断价值。结果:感染组WBC、CRP以及血清miR-146a 、miR-155水平高于未感染组(均P<0.05)。WBC、CRP、miR-146a、miR-155是 DKA患者合并感染的独立危险因素(均P<0.05)。DKA合并感染患者血清miR-146a、miR-155与WBC、CRP呈正相关,且血清miR-146a与miR-155呈正相关(均P<0.05)。血清miR-146a和miR-155对DKA合并感染均有一定的诊断价值,且两项联合的诊断价值更高(均P<0.05)。结论:DKA合并感染患者血清miR-146a、miR-155呈高水平,与WBC、CRP密切相关,对DKA合并感染具有较高诊断价值。
Abstract:
Objective:To detect serum microRNA(miR)-146a and miR-155 levels in patients with diabetic ketoacidosis(DKA)complicated with infection,and analyze their significance.Methods:A total of 76 DKA patients with infection(infection group)and 60 DKA patients without infection(uninfected group)were enrolled.Within 24 hours after admission,laboratory biochemical indicators were detected,and the serum levels of miR-146a and miR-155 were determined by RT-qPCR.The influencing factors of DKA complicated with infection,the correlation of serum miR-146a,miR-155,white blood cell count(WBC)and C-reactive protein(CRP)in DKA patients,and the diagnostic value of serum miR-146a and miR-155 for DKA complicated with infection were analyzed.Results: The levels of WBC,CRP,and serum miR-146a and miR-155 in the infection group were higher than those in the non-infection group(all P<0.05).WBC,CRP,miR-146a and miR-155 were independent risk factors for DKA complicated with infection(all P<0.05).Serum miR-146a and miR-155 were positively correlated with WBC and CRP,and serum miR-146a was positively correlated with miR-155 in DKA patients with infection(all P<0.05).Both serum miR-146a and miR-155 had certain diagnostic value for DKA complicated with infection,and the combination of the two had a higher diagnostic value(all P<0.05).Conclusion:The serum levels of miR-146a and miR-155 in DKA patients complicated with infection are high and closely related to WBC and CRP,which have high diagnostic value for DKA complicated with infection.
