摘要:
目的:探究局部注射富血小板血浆(PRP)联合水凝胶敷料对糖尿病足溃疡患者创面修复的影响及安全性。方法:将糖尿病足溃疡患者80例随机分为观察组(43例,接受PRP局部注射联合水凝胶敷料治疗)和对照组(37例,接受常规治疗)。比较两组患者的治疗效果、创面血流灌注量、细菌定量、治疗前后创面组织生化指标[血管内皮生长因子(VEGF)和透明质酸(HA)]、创面愈合情况以及并发症发生率。结果:观察组治疗总有效率高于对照组(P<0.05)。治疗后,两组创面血流灌注量、创面组织VEGF和HA水平高于治疗前,细菌定量低于治疗前,且观察组变化幅度大于对照组(均P<0.05)。观察组在肉芽组织生长厚度、肉芽组织覆盖率、创面缩小率、创面愈合时间和植皮时间方面优于对照组(均P<0.05)。两组并发症总发生率比较无统计学差异(P>0.05)。结论:局部注射PRP联合水凝胶敷料治疗能显著促进糖尿病足溃疡患者创面愈合,提升创面修复的生化环境,具有较高的安全性。
Abstract:
Objective:To investigate the effect and safety of local injection of platelet-rich plasma(PRP)combined with hydrogel dressing on wound repair in patients with diabetic foot ulcer.Methods:A total of 80 patients with diabetic foot ulcer were randomly divided into observation group(43 cases,treated with PRP local injection combined with hydrogel dressing)and control group(37 cases,treated with routine treatment).The therapeutic effect,wound blood perfusion volume,bacterial quantification,biochemical indexes of wound tissue(VEGF and HA)before and after treatment,wound healing,and incidence of complications were compared between the two groups.Results: The total effective rate in observation group was higher than that in control group(P<0.05).After treatment,the blood perfusion volume and the levels of VEGF and HA in wound tissue of the two groups were higher than those before treatment,and the bacterial quantification was lower than that before treatment,and the changes in observation group were larger than those in control group(all P<0.05).The granulation tissue growth thickness,granulation tissue coverage rate,wound reduction rate,wound healing time and skin grafting time in observation group were better than those in control group(all P<0.05).There was no significant difference in the total incidence of complications between two groups(P>0.05).Conclusion:Local injection of PRP combined with hydrogel dressing can significantly promote the wound healing of patients with diabetic foot ulcer,improve the biochemical environment for wound repair,and has high safety.
