摘要:
目的:探讨血清神经元特异性烯醇化酶(NSE)和D-二聚体(D-D)在高血压脑出血(HICH)术后神经功能障碍患者中的表达水平及检测意义。方法:选取HICH患者80例为研究对象,根据患者术后是否发生神经功能障碍分为预后良好组(48例)和预后不良组(32例)。比较两组患者临床资料和术前及术后第1、3、5、7、10、14天NSE和D-D表达水平。采用Logistic回归分析HICH患者术后发生神经功能障碍的影响因素。结果:预后不良组出血量多于预后良好组(P<0.05)。术后第3、5、7、10、14天预后良好组NSE水平低于预后不良组(均P<0.05)。术后第5、7、10、14天预后良好组D-D水平低于预后不良组(均P<0.05)。NSE、D-D水平是HICH患者术后发生神经功能障碍的独立影响因素(均P<0.05)。结论:HICH术后发生神经功能障碍患者,血清NSE、D-D表达水平升高。NSE、D-D水平是HICH患者术后发生神经功能障碍的独立影响因素。
Abstract:
Objective:To investigate the expression levels and significance of serum neuron-specific enolase(NSE)and D-dimer(D-D)in patients with neurological dysfunction after surgery of hypertensive intracerebral hemorrhage(HICH).Methods: Eighty patients with HICH were divided into a good prognosis group(48 cases)and a poor prognosis group(32 cases)according to whether they had neurological dysfunction after surgery.The clinical data and expression levels of NSE and D-D were compared between the two groups before and after surgery at 1,3,5,7,10 and 14 days.Logistic regression was used to analyze the influencing factors of postoperative neurological dysfunction in patients with HICH.Results: The amount of blood loss in poor prognosis group was higher than that in good prognosis group(P<0.05).At 3,5,7,10 and 14 days after surgery,NSE levels in good prognosis group were lower than those in poor prognosis group(all P<0.05).The levels of D-D in the good prognosis group were lower than those in the poor prognosis group at 5,7,10 and 14 days after surgery(all P<0.05).NSE and D-D levels were independent influencing factors for postoperative neurological dysfunction in patients with HICH(all P<0.05).Conclusion:The expression levels of serum NSE and D-D are increased in patients with neurological dysfunction after surgery of HICH.NSE and D-D levels are independent factors of postoperative neurological dysfunction in patients with HICH.
