摘要:
目的:探讨并发急性肺栓塞(APE)的非小细胞肺癌(NSCLC)患者血清碱性成纤维细胞生长因子(bFGF)、微小核糖核酸-1233(miR-1233)、血细胞参数的变化与危险分层及短期预后的关系。方法:采取病例对照研究方法,选取110例肺癌合并肺栓塞患者作为栓塞组、另外选取同期收治的肺癌但未合并肺栓塞的患者110例作为对照组,对比两组患者的血清bFGF、miR-1233、红细胞分布宽度(RDW)、血小板分布宽度(PDW)、血小板平均体积(MPV)、血小板淋巴细胞比率(PLR)、中性粒细胞淋巴细胞比率(NLR)水平,并按照不同危险分层将栓塞组进行分层对比,采用Logistic回归模型分析各项指标与急性肺栓塞的NSCLC患者的相关性。结果:栓塞组患者的bFGF、miR-1233、RDW、MPV、PLR、NLR水平均显著高于对照组,差异具有统计学意义(均P<0.05); 高风险组患者的bFGF、miR-1233、RDW、MPV、PLR、NLR水平均显著高于中低风险组患者,差异具有统计学意义(均P<0.05); 死亡组患者的bFGF、miR-1233、RDW、MPV、PLR、NLR水平均显著高于生存组患者,差异具有统计学意义(均P<0.05)。Logistic回归模型结果显示:ALB降低、NT-proBNP升高、合并房颤、合并肺部感染、bFGF升高、miR-1233升高、PLR升高、NLR升高是NSCLC合并肺栓塞患者不良预后结局的独立危险因素(均P<0.05)。结论:bFGF、miR-1233及血细胞参数与NSCLC发生肺栓塞有关,并且与患者治疗的不良预后结局关系密切。
Abstract:
Objective:To investigate the relationship between the changes of serum basic fibroblast growth factor(bFGF),microRNA-1233(miR-1233),blood cell parameters and risk stratification,short-term prognosis in non-small cell lung cancer(NSCLC)patients with acute pulmonary embolism(APE).Methods:A case-control study was conducted.110 patients with lung cancer complicated with APE were selected as the embolism group,and 110 patients with lung cancer but without APE were selected as the control group.The levels of serum bFGF,miR-1233,red blood cell distribution width(RDW),platelet distribution width(PDW),mean platelet volume(MPV),platelet lymphocyte ratio(PLR),and neutrophil lymphocyte ratio(NLR)were compared between the two groups,and the embolism group was stratified and compared according to different risk stratification.Logistic regression model was used to analyze the correlation between each index and NSCLC patients with APE.Results:The levels of bFGF,miR-1233,RDW,MPV,PLR and NLR in the embolism group were significantly higher than those in the control group(all P<0.05).The levels of bFGF,miR-1233,RDW,MPV,PLR and NLR in the high-risk group were significantly higher than those in the low-and medium-risk group(all P<0.05).The levels of bFGF,miR-1233,RDW,MPV,PLR and NLR in the death group were significantly higher than those in the survival group(all P<0.05).Logistic regression model results showed that decreased ALB,increased NT-proBNP,complicated with atrial fibrillation,complicated with pulmonary infection,increased bFGF,increased miR-1233,increased PLR,and increased NLR were independent risk factors for poor prognosis of NSCLC patients with APW(all P<0.05).Conclusion:BFGF,miR-1233,and blood cell parameters are associated with the occurrence of pulmonary embolism in NSCLC,and are closely related to the adverse prognosis of patient treatment.
