摘要:
目的:探讨多囊卵巢综合征(PCOS)不孕患者血清促肾上腺皮质激素(ACTH)、抗苗勒管激素(AMH)、胰岛素抵抗指数(HOMA-IR)水平变化,及其与负性情绪、健康相关生活质量的相关性。方法: 选取80例首次确诊为PCOS不孕患者作为观察组,另选取同期80例非PCOS不孕患者作为对照组。比较两组基线资料、性激素指标[黄体生成素(LH)、雌二醇(E2)、促卵泡素(FSH)、催乳素(PRL)]、HOMA-IR、ACTH、AMH水平及焦虑自评量表(SAS)评分、抑郁自评量表(SDS)、修订版多囊卵巢综合征健康相关生活质量问卷(MPCOSQ)评分; 分析HOMA-IR、ACTH、AMH水平与SAS、SDS、MPCOSQ评分的关系。结果:观察组空腹血糖、LH、E2、ACTH、AMH、HOMA-IR水平均高于对照组,PRL水平低于对照组,差异有统计学意义(均P<0.05)。观察组SAS、SDS、MPCOSQ评分均高于对照组,差异有统计学意义(均P<0.05)。PCOS不孕患者ACTH、AMH、HOMA-IR水平与LH、E2及SAS、SDS、MPCOSQ评分均呈正相关(均P<0.05),与PRL均呈负相关(P<0.05)。线性回归分析显示:ACTH、AMH、HOMA-IR均正向影响PCOS不孕患者的SAS、SDS评分(均P<0.05),ACTH、HOMA-IR均正向影响PCOS不孕患者的MPCOSQ评分(均P<0.05)。结论:PCOS不孕患者血清HOMA-IR、ACTH、AMH水平异常升高,且与SAS、SDS、MPCOSQ评分均呈正相关,HOMA-IR、ACTH、AMH或许可以作为反映PCOS不孕患者患者心理状态和健康相关生活质量的生物学指标。
Abstract:
Objective:To investigate the changes of serum adrenocorticotropic hormone(ACTH),anti-Mullerian hormone(AMH)and Insulin Resistance Index(HOMA-IR)in infertile women with polycystic ovary syndrome(PCOS),and their correlation with negative emotions and health-related quality of life.Methods:A total of 80 cases infertile patients with PCOS first diagnosed and treated were selected as the observation group,and 80 infertile patients without PCOS during the same period were selected as the control group.The clinical data,sex hormones(luteinizing hormone [LH],estradiol [E2],follicle stimulating hormone [FSH],prolactin [PRL]),HOMA-IR,ACTH,AMH,self-rating anxiety scale(SAS)score,self-rating depression scale(SDS)and modified polycystic ovary syndrome health related quality of life questionnaire(MPCOS)were compared between the two groups.The relationship between HOMA-IR,ACTH,AMH and SAS,SDS,MPCOSQ scores was analyzed.Results:The levels of fasting blood glucose,LH,E2,ACTH,AMH and HOMA-IR in the observation group were higher than those of the control group,and the levels of PRL was lower than those of the control group(all P<0.05).The SAS,SDS and MPCOSQ scores of the observation group were higher than those of the control group(all P<0.05).ACTH,AMH,HOMA-IR were positively correlated with LH,E2,SAS,SDS,MPCOSQ scores,and negatively correlated with PRL(all P<0.05).Linear regression analysis showed that ACTH,AMH,and HOMA-IR all positively affected the SAS and SDS scores of PCOS,ACTH and HOMA-IR all positively affect the MPCOSQ scores of PCOS(all P<0.05).Conclusion:The levels were increased of HOMA-IR,ACTH,and AMH in infertile patients with PCOS,and were positively correlated with SAS,SDS,and MPCOSQ scores.HOMA-IR,ACTH and AMH may be used as biological indicators to reflect the psychological state and health-related quality of life of PCOS infertile patients.
