摘要:
目的:探讨食管鳞癌组织中c-myc、神经鞘胚素(Artemin)表达情况及其与患者临床病理特征和预后的关系。方法:收集接受手术治疗的食管鳞癌患者75例,采集食管鳞癌组织及癌旁组织标本。采用免疫组化(SP法)检测标本组织中c-myc、Artemin的表达情况。术后随访3年,记录患者生存情况。分析c-myc、Artemin表达与患者临床病理特征的关系。采用Spearman法分析c-myc与Artemin表达的相关性。采用多因素Cox回归分析食管鳞癌患者预后的影响因素。结果:食管鳞癌组织中c-myc阳性表达率高于癌旁组织(62.67%与42.67%,P=0.014)。食管鳞癌组织中Artemin阳性表达率高于癌旁组织(78.67%与44.00%,P=0.000)。食管鳞癌组织中c-myc表达与肿瘤的分化程度、浸润深度以及TNM分期有关(均P<0.05)。食管鳞癌组织中Artemin表达与肿瘤的浸润深度、淋巴结转移以及TNM分期有关(均P<0.05)。食管鳞癌组织中c-myc与Artemin的表达呈正相关(r=0.406,P=0.012)。食管鳞癌患者预后与肿瘤分化程度、浸润深度、淋巴结转移、TNM分期、c-myc表达以及Artemin表达有关(均P<0.05)。有淋巴结转移、肿瘤低分化、TNM分期Ⅲ期、c-myc表达阳性、Artemin表达阳性是食管鳞癌患者预后的独立危险因素(均P<0.05)。结论:c-myc、Artemin在食管鳞癌组织中过度表达。c-myc与肿瘤分化程度、浸润深度以及TNM分期有关。Artemin与肿瘤浸润深度、淋巴结转移以及临床分期有关。两者阳性表达均是食管鳞癌患者预后的独立危险因素。
Abstract:
Objective:To investigate the expressions of c-myc and Artemin in esophageal squamous cell carcinoma tissues and their relationship with clinicopathological features and prognosis of patients.Methods:Seventy-five patients with esophageal squamous cell carcinoma who underwent surgical treatment were collected,and the samples of esophageal squamous cell carcinoma tissues and adjacent tissues were collected.Immunohistochemistry(SP method)was used to detect the expression of c-myc and Artemin in specimens.The patients were followed up for 3 years,and the survival of patients was recorded.The relationship between the expressions of c-myc,Artemin and the clinicopathological features of patients was analyzed.Spearman method was used to analyze the correlation between c-myc and Artemin expression.Multivariate Cox regression was used to analyze the prognostic factors of patients with esophageal squamous cell carcinoma.Results:The positive expression rate of c-myc in esophageal squamous cell carcinoma tissues was significantly higher than that in adjacent normal tissues(62.67% vs.42.67%,P=0.014).The positive expression rate of Artemin in esophageal squamous cell carcinoma tissues was higher than that in adjacent normal tissues(78.67% vs.44.00%,P=0.000).The expression of c-myc in esophageal squamous cell carcinoma tissues was related to the degree of differentiation,depth of invasion and TNM stage(all P<0.05).The expression of Artemin in esophageal squamous cell carcinoma tissues was related to the depth of invasion,lymph node metastasis and TNM stage(all P<0.05).There was a positive correlation between the expression of c-myc and Artemin in esophageal squamous cell carcinoma tissues(r=0.406,P=0.012).The prognosis of patients was related to the degree of tumor differentiation,depth of invasion,lymph node metastasis,TNM stage,c-myc expression and Artemin expression(all P<0.05).Lymph node metastasis,poor differentiation,TNM stage Ⅲ,c-myc positive expression and Artemin positive expression were independent risk factors for the prognosis of patients with esophageal squamous cell carcinoma(all P<0.05).Conclusion:Esophageal squamous cell carcinoma tissues have overexpression of c-myc and Artemin.The c-myc is related to the degree of tumor differentiation,depth of invasion and TNM stage.The Artemin is related to the depth of tumor invasion,lymph node metastasis and clinical stage.Both positive expression of c-myc and Artemin are independent risk factors for the prognosis of esophageal squamous cell carcinoma patients.
