摘要:
目的:探讨血清铁参数和血小板参数与老年人群缺铁性贫血(IDA)及严重程度的关系及其诊断价值。方法:选取老年IDA患者95例为研究组,根据贫血严重程度分为重度组(17例)、中度组(42例)和轻度组(36例)。另选取同期老年体检健康者95例作为对照组。检测并比较研究组与对照组、不同贫血严重程度IDA患者血清铁参数[血清铁(SI)、血清铁蛋白(SF)、血清可溶性转铁蛋白受体(sTfR)]和血小板参数[血小板计数(PLT)、血小板分布宽度(PDW)、平均血小板体积(MPV)]。采用受试者工作特征(ROC)曲线分析血清铁参数和血小板参数对老年IDA的诊断价值; 采用Spearman法分析血清铁参数和血小板参数与老年IDA患者贫血严重程度的相关性。结果:研究组血清SI、SF水平低于对照组,sTfR水平高于对照组(均P<0.05)。研究组PLT、PDW、MPV高于对照组(均P<0.05)。重度组血清SI、SF水平低于中度组及轻度组,中度组血清SI、SF水平低于轻度组(均P<0.05)。重度组血清sTfR水平高于中度组及轻度组,中度组血清sTfR水平高于轻度组(均P<0.05)。重度组PLT、PDW、MPV高于中度组及轻度组,中度组PLT、PDW、MPV高于轻度组(均P<0.05)。ROC曲线分析显示,血清SI、SF、sTfR、PLT、PDW和MPV诊断老年IDA的曲线下面积(AUC)为0.947、0.927、0.892、0.748、0.738、0.751(均P<0.05)。Spearman相关性分析显示,血清SI、SF与老年IDA患者贫血严重程度呈负相关(r=-0.723、-0.786,均P<0.05),sTfR、PLT、PDW和MPV与老年IDA患者贫血严重程度呈正相关(r=0.753、0.571、0.704、0.636,均P<0.05)。结论:老年IDA患者血清铁和血小板参数存在不同程度变化,且与患者贫血严重程度有关,对老年IDA有一定的诊断价值。
Abstract:
Objective:To investigate the relationship of serum iron parameters,platelet parameters with iron deficiency anemia(IDA)and severity in elderly and their diagnostic value.Methods:A total of 95 elderly patients with IDA were enrolled as study group.They were divided into severe group(17 cases),moderate group(42 cases)and mild group(36 cases)according to the severity of anemia.A total of 95 healthy elderly people were selected as the control group.Serum iron parameters(serum iron [SI],serum ferritin [SF],serum soluble transferrin receptor [sTfR])and platelet parameters(platelet count[PLT],platelet distribution width [PDW],mean platelet volume [MPV])were detected and compared between the study group and the control group,and between IDA patients with different severity of anemia.ROC curve was used to analyze the diagnostic value of serum iron parameters and platelet parameters for IDA in the elderly.Spearman method was used to analyze the correlation between serum iron parameters,platelet parameters and the severity of anemia in elderly patients with IDA.Results:The levels of serum SI and SF in the study group were lower than those in the control group,and the level of sTfR was higher than that in the control group(all P<0.05).PLT,PDW and MPV in the study group were higher than those in the control group(all P<0.05).The serum levels of SI and SF in the severe group were lower than those in the moderate and mild groups,and the serum levels of SI and SF in the moderate group were lower than those in the mild group(all P<0.05).The severe group had a significantly higher serum sTfR level than the moderate and mild groups,and the moderate group had a significantly higher serum sTfR level than the mild group(all P<0.05).PLT,PDW and MPV in the severe group were higher than those in the moderate and mild groups,and PLT,PDW and MPV in the moderate group were higher than those in the mild group(all P<0.05).ROC curve analysis showed that the AUC of SI,SF,sTfR,PLT,PDW and MPV for the diagnosis of IDA in the elderly were 0.947,0.927,0.892,0.748,0.738 and 0.751(all P<0.05).Spearman correlation analysis showed that serum SI and SF were negatively correlated with the severity of anemia in elderly IDA patients(r=-0.723,-0.786,both P<0.05); sTfR,PLT,PDW and MPV were positively correlated with the severity of anemia in elderly IDA patients(r=0.753,0.571,0.704,0.636,all P<0.05).Conclusion:Serum iron and platelet parameters change in different degrees in elderly patients with IDA,which are related to the severity of anemia and have certain diagnostic value for IDA in the elderly.
