摘要:
目的:探讨2型糖尿病(T2DM)合并桥本甲状腺炎(HT)患者血清微小RNA-22-5p(miR-22-5p)、25羟维生素D水平变化及其预测价值。方法:选取T2DM患者96例,根据是否合并HT分为T2DM组(50例)和T2DM+HT组(46例)。另选择同期体检健康者40例为对照组。比较三组一般资料、空腹血糖(FBG)、糖化血红蛋白(HbA1c)、甲功五项[游离三碘甲状腺原氨酸(FT3)、游离甲状腺素(FT4)、促甲状腺激素(TSH)、甲状腺过氧化物酶抗体(TPOAb)、甲状腺球蛋白抗体(TGAb)]、血清炎症因子[高敏C反应蛋白(hs-CRP)、肿瘤坏死因子α(TNF-α)、白细胞介素-6(IL-6)和IL-10]、25羟维生素D和miR-22-5p。采用Spearman法分析甲功五项、血清炎症因子与miR-22-5p及25羟维生素D的相关性。绘制受试者工作特征(ROC)曲线分析miR-22-5p、25羟维生素D对T2DM合并HT的预测价值。结果:三组一般资料比较差异无统计学意义(均P>0.05)。与对照组比较,T2DM组FBG、HbA1c、hs-CRP、TNF-α、IL-6和miR-22-5p升高,25羟维生素D降低(均P<0.05); T2DM+HT组FBG、HbA1c、FT3、FT4、TSH、TPOAb、TGAb、hs-CRP、TNF-α、IL-6和miR-22-5p升高,IL-10和25羟维生素D降低(均P<0.05)。T2DM组FT3、FT4、TSH、TPOAb、TGAb、hs-CRP、TNF-α、IL-6、IL-10与miR-22-5p、25羟维生素D无相关性(均P>0.05)。T2DM+HT组FT3、FT4、TSH、IL-10与miR-22-5p、25羟维生素D无相关性(均P>0.05),TPOAb、TGAb、hs-CRP、TNF-α、IL-6与25羟维生素D呈负相关(均P<0.05),与miR-22-5p呈正相关(均P<0.05)。25羟维生素D、miR-22-5p以及两者联合检测预测T2DM合并HT的曲线下面积(AUC)分别为0.774、0.842、0.915,联合检测的预测价值较高。结论:T2DM合并HT患者25羟维生素D水平降低,miR-22-5p表达升高,两者与TPOAb、TGAb、hs-CRP、TNF-α、IL-6具有相关性,对T2DM合并HT具有较好的预测价值。
Abstract:
Objective:To investigate the changes and predictive value of serum microRNA-22-5p(miR-22-5p)and 25-hydroxyvitamin D levels in patients with type 2 diabetes mellitus(T2DM)complicated with Hashimoto's thyroiditis(HT).Methods:A total of 96 patients with T2DM were divided into T2DM group(50 cases)and T2DM+HT group(46 cases)according to the presence or absence of HT.Another 40 healthy people were selected as the control group.The general data,fasting blood glucose(FBG),glycosylated hemoglobin(HbA1c),five items of thyroid function(free triiodothyronine [FT3],free thyroxine [FT4],thyroid stimulating hormone [TSH],thyroid peroxidase antibody [TPOAb],thyroglobulin antibody [TGAb])and serum inflammatory factors(high C-reactive protein [hs-CRP],tumor necrosis factor-α [TNF-α],interleukin-6 [IL-6] and IL-10),25-hydroxyvitamin D and miR-22-5p were compared among the three groups.Spearman method was used to analyze the correlation between five items of thyroid function,serum inflammatory factors and miR-22-5p,25-hydroxyvitamin D.ROC curve was drawn to analyze the predictive value of miR-22-5p and 25-hydroxyvitamin D for T2DM complicated with HT.Results:There were no significant differences in general data among the three groups(all P>0.05).Compared with the control group,the levels of FBG,HbA1c,hs-CRP,TNF-α,IL-6 and miR-22-5p were increased,and 25-hydroxyvitamin D was decreased in the T2DM group(all P<0.05).The levels of FBG,HbA1c,FT3,FT4,TSH,TPOAb,TGAb,hs-CRP,TNF-α,IL-6 and miR-22-5p were increased,while IL-10 and 25-hydroxyvitamin D were decreased in the T2DM+HT group( all P<0.05).There was no correlation between FT3,FT4,TSH,TPOAb,TGAb,hs-CRP,TNF-α,IL-6,IL-10 and miR-22-5p,25-hydroxyvitamin D in T2DM group(all P>0.05).In T2DM+HT group,FT3,FT4,TSH and IL-10 were not correlated with miR-22-5p and 25-hydroxyvitamin D(all P>0.05),while TPOAb,TGAb,hs-CRP,TNF-α and IL-6 were negatively correlated with 25-hydroxyvitamin D and positively correlated with miR-22-5p(all P<0.05).The AUC of 25-hydroxyvitamin D,miR-22-5p and their combined detection in predicting T2DM complicated with HT was 0.774,0.842 and 0.915 respectively,and the combined detection had a higher predictive value.Conclusion:T2DM patients complicated with HT have a decreased level of 25-hydroxyvitamin D and an increased expression of miR-22-5p,which are correlated with TPOAb,TGAb,hs-CRP,TNF-α and IL-6,and have a good predictive value for T2DM complicated with HT.
