摘要:
目的:探究抑郁症患者血清胶质细胞源性神经营养因子(GDNF)、胰岛素样生长因子1(IGF-1)水平变化及其与睡眠障碍的关系。方法:选取抑郁症患者120例为抑郁症组,选取体检健康者120例为对照组。采用酶联免疫吸附法测定所有受试者血清GDNF、IGF-1水平。对所有受试者进行匹茨堡睡眠质量指数量表(PSQI)评分,根据PSQI评分结果将抑郁症患者分为无睡眠障碍组(22例)和有睡眠障碍组(98例),其中轻度、中度、重度睡眠障碍组分别为28例、43例、27例。比较抑郁症组与对照组血清GDNF、IGF-1水平及PSQI评分,以及不同临床特征抑郁症患者血清GDNF、IGF-1水平。比较无睡眠障碍组与有睡眠障碍组抑郁症患者以及不同严重程度睡眠障碍抑郁症患者血清GDNF、IGF-1水平及PSQI评分。分析抑郁症伴睡眠障碍患者血清GDNF、IGF-1水平与PSQI评分的相关性,抑郁症患者血清GDNF、IGF-1水平对睡眠障碍发生的预测价值,以及抑郁症患者睡眠障碍发生的影响因素。结果:抑郁症组血清GDNF、IGF-1水平低于对照组,PSQI评分高于对照组(均P<0.05)。抑郁症患者血清GDNF、IGF-1水平与自杀行为、发病次数、家族史无关(均P>0.05)。有睡眠障碍组血清GDNF、IGF-1水平低于无睡眠障碍组,PSQI评分高于无睡眠障碍组(均P<0.05)。轻度睡眠障碍组、中度睡眠障碍组和重度睡眠障碍组血清GDNF、IGF-1水平依次降低,PSQI评分依次升高(均P<0.05)。有睡眠障碍抑郁症患者血清GDNF与IGF-1水平呈正相关,血清GDNF、IGF-1水平均与PSQI评分呈负相关(均P<0.05)。抑郁症患者血清GDNF、IGF-1单项及两者联合预测睡眠障碍发生的曲线下面积(AUC)分别为0.767、0.754、0.854,其中血清GDNF、IGF-1各自单独预测的AUC低于两者联合预测的AUC(均P<0.05)。GDNF低水平、IGF-1低水平均是抑郁症患者发生睡眠障碍的独立危险因素(均P<0.05)。结论:有睡眠障碍抑郁症患者血清GDNF、IGF-1呈低水平,且两者与睡眠障碍严重程度有关。血清GDNF、IGF-1对抑郁症患者睡眠障碍的发生有较高预测价值。
Abstract:
Objective:To investigate the changes of serum glial cell derived neurotrophic factor(GDNF)and insulin like growth factor 1(IGF-1)levels in patients with depression and their relationship with sleep disorders.Methods:A total of 120 patients with depression were selected as the depression group and 120 healthy volunteers as the control group.Serum GDNF and IGF-1 levels were determined by enzyme-linked immunosorbent assay.All subjects were scored by PSQI and divided into no sleep disorder group(22 cases)and sleep disorder group(98 cases)according to PSQI score,and there were 28 cases of mild,43 cases of moderate and 27 cases of severe sleep disorder.Serum GDNF and IGF-1 levels and PSQI scores between depression group and control group,serum GDNF and IGF-1 levels of depression patients with different clinical characteristics were compared.Serum GDNF,IGF-1 levels and PSQI scores of depression patients in no sleep disorder group and sleep disorder group,depression patients with different severity of sleep disorder were compared.The correlation of serum GDNF,IGF-1 levels and PSQI scores in depressed patients with sleep disorders,the predictive value of serum GDNF and IGF-1 levels in depressed patients on the occurrence of sleep disorders,and the factors affecting the occurrence of sleep disorders in depressed patients were analyzed.Results:Serum GDNF and IGF-1 levels in depression group were significantly lower than those in control group,and PSQI score was significantly higher than that in control group(all P<0.05).The serum levels of GDNF and IGF-1 in patients with depression were not associated with suicidal behavior,frequency of onset,and family history(all P>0.05).The levels of serum GDNF and IGF-1 in sleep disorder group were significantly lower than those in no sleep disorder group,and the PSQI score was significantly higher than that in no sleep disorder group(all P<0.05).The levels of serum GDNF and IGF-1 in mild sleep disorder group,moderate sleep disorder group and severe sleep disorder group were decreased successively,while the PSQI score was increased successively(all P<0.05).Serum GDNF and IGF-1 levels in depressive patients with sleep disorders were positively correlated,and serum GDNF and IGF-1 levels were negatively correlated with PSQI scores(all P<0.05).The AUC predicted by serum GDNF,IGF-1,and their combination for the occurrence of sleep disorder in patients with depression were 0.767,0.754 and 0.854,respectively,and the AUC predicted by serum GDNF and IGF-1 separately was lower than the AUC predicted by their combination(all P<0.05).Low levels of GDNF and IGF-1 were independent risk factors for sleep disorders in patients with depression(all P<0.05).Conclusion:The serum levels of GDNF and IGF-1 in depressive patients with sleep disorder were low,and the more serious the sleep disorder was,the lower the levels of both were.The serum levels of GDNF and IGF-1 had a high predictive value on the occurrence of sleep disorder in depressive patients.
