摘要:
目的:探讨血清胎盘蛋白-13(PP-13)、β-人绒毛膜促性腺激素(β-HCG)、分泌型卷曲相关蛋白5(SFRP5)及血清胎盘生长因子(PLGF)检测对妊娠期糖尿病不良妊娠结局的诊断价值。方法:选择118例妊娠期糖尿病孕妇作为观察组,另选69例同期孕检健康孕妇作为对照组,收集两组孕妇的一般资料包括年龄、孕周、体重等,检测两组孕妇血清PP-13、β-HCG、SFRP5、PLGF、孕酮(P)、妊娠相关血浆蛋白A(PAPP-A)、血管内皮生长因子(VEGF)水平,比较两组患者的一般资料,采用Logistic多因素回归分析孕妇妊娠结局的危险因素。结果:观察组的PP-13、SFRP5、PLGF、P、PAPP-A、VEGF明显低于对照组,观察组的β-HCG明显高于对照组,差异对比均有统计学意义(均P<0.05)。多因素Logistic回归分析结果显示,血清PP-13、β-HCG、SFRP5、PLGF、P、PAPP-A、VEGF水平均是妊娠期糖尿病患者发生不良妊娠结局的独立危险因素(均P<0.05)。血清PP-13、β-HCG、SFRP5、PLGF联合检测诊断GDM孕妇不良妊娠结局的效能明显高于单独检测,单独血清PP-13、β-HCG、SFRP5、PLGF检测及联合检测的AUC分别为0.873、0.748、0.859、0.838、0.930,预测价值较为理想。结论:血清PP-13、β-HCG、SFRP5与PLGF水平与妊娠期糖尿病的控制程度有一定的联系,对于存在危险因素的孕妇需要加强产检,尽早发现、尽早治疗,从而减少不良妊娠结局的发生,改善母婴预后情况。
