摘要:
目的:评价腹腔镜手术治疗老年子宫内膜癌(EC)的疗效、安全性及对患者应激、疼痛和术后恢复的影响。方法:选取接受手术治疗的老年EC患者160例,根据手术方式不同分为开腹组和腹腔镜组,各80例。评价两组的治疗有效率、围手术期指标、并发症发生情况、术后疼痛、应激状态、生活质量等。结果:腹腔镜组和开腹组临床总有效率分别为91.25%和81.25%,组间比较差异无统计学意义(P>0.05); 两组术中出血量、术后肛门排气时间、术后引流时间及住院天数比较,腹腔镜组均更优(均P<0.05); 两组术中输血率、并发症总发生率比较,腹腔镜组更低(均P<0.05); 腹腔镜组术后24 h疼痛VAS评分明显低于开腹组(P<0.05); 两组术后1 d血清皮质醇和肾上腺素水平高于术前,SDS和SAS评分低于术前,且腹腔镜组术后1 d上述指标均优于开腹组(均P<0.05); 腹腔镜组患者术后1个月和术后6个月时的癌症治疗功能性量表(FACT-L)各领域评分均高于同时期开腹组(均P<0.05)。结论:腹腔镜手术治疗老年子宫内膜癌具有术中出血少、患者疼痛轻、术后恢复快、并发症少及可缓解患者应激反应等优势,提高患者生活质量。
Abstract:
Objective:To evaluate the efficacy and safety of laparoscopic surgery for elderly endometrial carcinoma(EC)and its impact on patients' stress,pain and postoperative recovery.Methods:A total of 160 elderly EC patients who underwent surgical were selected and divided into open group and laparoscopic group according to different surgical methods,with 80 cases each.The therapeutic effect,complications,postoperative pain,stress status,quality of life of the two groups were evaluated and compared statistically.Results:The total effective rate of laparoscopic group and open group was 91.25% and 81.25% respectively,and there was no difference between the two groups(P>0.05).The intraoperative blood loss,postoperative anal exhaust time,postoperative drainage time and hospital stay in the laparoscopic group were better than those in the control group(all P<0.05).The intraoperative blood transfusion rate and the total incidence of complications in the laparoscopic group were lower than those in the control group(all P<0.05).The VAS score of pain in the laparoscopic group was significantly lower than that in the open group at 24 hours after operation(P<0.05).The serum levels of cortisol and adrenaline in the two groups at 1 day after operation were higher than those before operation,and the SDS and SAS scores were lower than those before operation,and the above indexes in the laparoscopic group were better than those in the open group at 1 day after operation(all P<0.05).The scores of FACT-L in all domains in the laparoscopic group were higher than those in the open group at 1 month and 6 months after operation(all P<0.05).Conclusion:Laparoscopic surgery for the treatment of elderly endometrial cancer has the advantages of less intraoperative bleeding,less pain,faster postoperative recovery,less complications,and can relieve the stress response of patients,and improve the quality of life of patients.
