摘要:
目的:探讨血清基质金属蛋白酶-9(MMP-9)、二胺氧化酶(DAO)、Toll样受体9(TLR9)与急性胰腺炎(AP)患者疾病严重程度及肠黏膜屏障损伤的相关性。方法:选择AP患者82例为观察组,根据亚特兰大分类分为三个亚组:轻度AP组(32例)、中度AP组(26例)和重度AP组(24例)。选择同期体检健康者50例为对照组。比较观察组与对照组血清MMP-9、DAO、TLR9水平及肠黏膜屏障损伤指标。比较不同严重程度AP患者入院第1、2、3、7天血清MMP-9、DAO、TLR9水平及肠黏膜屏障损伤指标。分析血清MMP-9、DAO、TLR9水平与AP严重程度及肠黏膜屏障损伤指标的相关性。结果:观察组血清MMP-9、DAO、TLR9、内毒素、D-乳酸水平及乳果糖/甘露醇比值较对照组高(均P<0.05)。与轻度AP组比较,中度和重度AP组血清MMP-9、DAO、TLR9、内毒素、D-乳酸水平及乳果糖/甘露醇比值升高(均P<0.05); 与中度AP组比较,重度AP组血清MMP-9、DAO、TLR9、内毒素、D-乳酸水平及乳果糖/甘露醇比值升高(均P<0.05)。与入院第1天比较,三组AP患者入院第2天血清MMP-9、DAO、TLR9、内毒素、D-乳酸水平及乳果糖/甘露醇比值升高,入院第3、7天血清MMP-9、DAO、TLR9、内毒素、D-乳酸水平及乳果糖/甘露醇比值下降(均P<0.05); 与入院第2天比较,三组AP患者第3、7天血清MMP-9、DAO、TLR9、内毒素、D-乳酸水平及乳果糖/甘露醇比值下降(均P<0.05); 与入院第3天比较,三组AP患者入院第7天血清MMP-9、DAO、TLR9、内毒素、D-乳酸水平及乳果糖/甘露醇比值下降(均P<0.05)。血清MMP-9、DAO、TLR9与急性胰腺炎严重程度及肠黏膜屏障损伤指标均呈正相关(P<0.05)。结论:急性胰腺炎患者血清MMP-9、DAO、TLR9水平存在异常,与患者疾病严重程度及肠黏膜屏障损伤呈正相关,可用于监测疾病严重程度及肠黏膜屏障损伤。
Abstract:
Objective:To investigate the association of serum matrix metalloproteinase-9(MMP-9),diamine oxidase(DAO),and Toll-like receptor 9(TLR9)with disease severity and mucosal barrier injury in patients with acute pancreatitis(AP).Methods:A total of 82 AP patients were chosen as observation group and were divided into three subgroups:mild AP group(32 cases),moderate AP group(26 cases)and severe AP group(24 cases).A total of 50 healthy subjects during the same period were selected as control group.The serum levels of MMP-9,DAO,TLR9 and intestinal mucosal barrier injury indexes were compared between observation group and control group.The serum levels of MMP-9,DAO,TLR9 and intestinal mucosal barrier injury indexes of AP patients with different severity on admission day 1,2,3 and 7 were compared between observation group and control group.The correlation between serum MMP-9,DAO,TLR9 and the severity of AP,intestinal mucosal barrier injury indexes was analyzed.Results:The serum levels of MMP-9,DAO,TLR9,endotoxin,D-lactate and lactulose/mannitol ratio in observation group were significantly higher than those in control group(all P<0.05).Compared with mild AP group,the serum levels of MMP-9,DAO,TLR9,endotoxin,D-lactate and lactilose/mannitol ratio in moderate and severe AP groups were significantly higher(all P<0.05); Compared with moderate AP group,the serum levels of MMP-9,DAO,TLR9,endotoxin,D-lactate and lactilose/mannitol ratio in severe AP group were significantly higher(all P<0.05).Compared with the 1st day after admission,the serum levels of MMP-9,DAO,TLR9,endotoxin,D-lactate and lactilose/mannitol ratio were significantly increased in three AP groups on 2nd day,and those were significantly decreased on the 3rd and 7th day after admission(all P<0.05).Compared with the 2nd day after admission,the levels of serum MMP-9,DAO,TLR9,endotoxin,D-lactate and lactilose/mannitol ratio were significantly decreased in three AP groups on the 3rd and 7th day(all P<0.05).Compared with the 3rd day,the levels of serum MMP-9,DAO,TLR9,endotoxin,D-lactate and lactilose/mannitol ratio were significantly decreased in three AP groups on the 7th day(all P<0.05).Serum MMP-9,DAO and TLR9 levels were positively correlated with the severity of AP and intestinal mucosal barrier injury indexes(all P<0.05).Conclusion:Serum MMP-9,DAO and TLR9 levels in AP patients are abnormal,which are positively correlated with disease severity and mucosal barrier injury,and can be used to predict the severity of disease and intestinal mucosal barrier injury.
