摘要:
目的:比较超声结合99mTc-MIBI SPECT/CT双时相显像不同图像采集方案对异位继发性甲状旁腺功能亢进症(SHPT)的术前诊断价值。方法:选择SHPT患者80例为观察组,选择同期疑似异位SHPT但经病理学检查排除为异位SHPT患者80例为对照组。所有患者术前行超声结合99mTc-MIBI SPECT/CT双时相法显像,图像采集方案包括双期平面像(方案一)、双期平面像及早期SPECT/CT(方案二)、双期平面像及延迟期SPECT/CT(方案三)、双期平面像及双期SPECT/CT(方案四)。比较四种方案术前诊断异位SHPT的结果。以手术病理结果为金标准,采用受试者工作特征(ROC)曲线分析四种方案对异位SHPT的诊断价值。结果:方案一、方案二、方案三和方案四诊断异位SHPT的阳性例数分别为81例、79例、81例和81例,四种方案术前诊断异位SHPT结果比较差异有统计学意义(均P<0.05)。ROC曲线分析结果显示,四种方案对异位SHPT均具有一定的诊断价值(均P<0.05),其中方案四的诊断价值最高。结论:在超声结合99mTc-MIBI SPECT/CT双时相显像过程中采用双期平面像及双期SPECT/CT方案对异位SHPT具有较高诊断价值。
Abstract:
Objective:To compare the value of different image acquisition schemes in preoperative diagnosis of ectopic secondary hyperparathyroidism(SHPT)in ultrasound combined with 99mTc-MIBI SPECT/CT dual-phase imaging.Methods:A total of 80 patients treated with ectopic SHPT were selected as the observation group,and 80 patients with suspected ectopic SHPT but excluded by pathological examination were selected as the control group.All patients underwent ultrasound combined with 99mTc-MIBI SPECT/CT dual-phase imaging before surgery.The image acquisition schemes were divided into four groups:biphasic plane image(scheme 1),biphasic plane image and early SPECT/CT(scheme 2),biphasic plane image and delayed SPECT/CT(scheme 3),biphasic plane image and biphasic SPECT/CT(scheme 4).The results of four schemes for preoperative diagnosis of ectopic SHPT was compared.Taking the surgical and pathological results as the gold standard,the ROC curve was used to analyze the diagnostic value of four schemes for ectopic SHPT.Results:The number of positive cases of ectopic SHPT in scheme 1,scheme 2,scheme 3 and scheme 4 were 81,79,81 and 81 respectively,and there were significant differences in the results of preoperative diagnosis of ectopic SHPT among the four schemes(all P<0.05).ROC curve analysis showed that all four schemes had certain diagnostic value for ectopic SHPT(all P<0.05),and scheme 4 had the highest diagnostic value.Conclusion:The use of dual phase plane image and dual phase SPECT/CT scheme in the process of ultrasound combined with 99mTc-MIBI SPECT/CT dual phase imaging has high diagnostic value for ectopic SHPT.
