摘要:
目的:分析早发冠心病(PCAD)患者载脂蛋白E(ApoE)基因多态性及其与冠状动脉病变严重程度关系。方法:以冠心病患者279例为研究对象,根据发病年龄不同分为早发组(91例)和迟发组(188例)。比较两组ApoE基因多态性,分析其与PCAD发生的关系。根据Gensini积分将早发组患者分为轻度组(39例)、中度组(29例)和重度组(23例),比较三组ApoE基因多态性并分析其对PCAD患者冠状动脉病变严重程度的评估价值。结果:早发组ApoE基因E3/E3型检出率高于迟发组(P<0.05),但两组ApoE基因E2/E2、E2/E3、E2/E4、E3/E4以及E4/E4型检出率比较差异无统计学意义(均P>0.05)。ApoE基因E3/E3型是影响PCAD发生的危险因素(P<0.05)。PCAD患者ApoE基因E3/E4型检出率随冠状动脉病变严重程度的加重而升高(P<0.05)。ApoE基因E2/E3、E3/E3和E3/E4型对PCAD患者冠状动脉病变严重程度具有一定的评估价值,且各基因型联合评估的曲线下面积(AUC)更高(均P<0.05)。结论:PCAD患者ApoE基因以E3/E3型为主,E3/E4型或与冠状动脉病变严重程度有关,且ApoE基因多态性分析对PCAD患者冠状动脉病变严重程度具有较好的评估价值。
Abstract:
Objective:To analyze the polymorphisms of human apolipoprotein E(ApoE)gene and their relationship with the severity of coronary artery lesions in patients with premature coronary artery disease(PCAD).Methods:A total of 279 patients with PCAD were enrolled as the research objects.According to different onset age,they were divided into early-onset group(91 cases)and late-onset group(188 cases).The polymorphisms of ApoE gene between two groups were compared,and their relationship with the occurrence of PCAD was analyzed.Patients in early-onset group were divided into mild group(39 cases),moderate group(29 cases)and severe group(23 cases)according to Gensini score,and ApoE gene polymorphisms were compared among the three groups,and their value in evaluating the severity of coronary artery lesions in patients with PCAD was analyzed.Results:The detection rate of E3/E3 ApoE gene in early-onset group was higher than that in late-onset group(P<0.05),and there was no significant difference in detection rates of E2/E2,E2/E3,E2/E4,E3/E4 and E4/E4 ApoE gene between two groups(all P>0.05).E3/E3 type of ApoE gene was a risk factor of PCAD(P<0.05).With the aggravation of coronary artery lesions,the detection rate of E3/E4 ApoE gene was increased(P<0.05).E2/E3,E3/E3 and E3/E4 ApoE genes had certain value in evaluating the severity of coronary artery lesions in patients with PCAD,and the AUC of the combined evaluation of each genotype was higher(all P<0.05).Conclusion:The polymorphism of ApoE gene is mainly on E3/E3 type in PCAD patients.E3/E4 type may be related to the severity of coronary artery lesions.ApoE gene polymorphism analysis has a good value in evaluating the severity of coronary artery lesions in PCAD patients.
