摘要:
目的:探究川崎病患儿心血管病变易感性与血清N末端B型脑钠肽原(NT-proBNP)、前白蛋白(PA)及C-反应蛋白(CRP)水平的相关性。方法:回顾性分析92例川崎病患儿的临床资料,依据是否出现心血管病变分为A组(49例,发生心血管病变)和B组(43例,未发生心血管病变)。另选取同期体检健康儿童40例作为对照组。比较三组血清NT-proBNP、PA、CRP水平。采用Pearson法分析川崎病患儿心血管病变与血清NT-proBNP、PA、CRP水平的相关性。采用Logistic回归模型分析川崎病患儿发生心血管病变的危险因素。结果:三组血清NT-proBNP、PA、CRP水平组间比较差异有统计学意义(P<0.05)。A组和B组血清NT-proBNP、CRP水平高于对照组,且A组高于B组(均P<0.05)。A组和B组血清PA水平低于对照组,且A组低于B组(均P<0.05)。Pearson相关性分析表明,川崎病患儿心血管病变与血清NT-proBNP、CRP水平呈正相关,与血清PA水平呈负相关(r=0.463、0.356、-0.391,均 P<0.05)。Logistic回归分析结果显示,血清NT-proBNP、PA水平是川崎病患儿发生心血管病变的独立危险因素(均P<0.05)。结论:川崎病患儿心血管病变易感性与血清NT-proBNP、PA及CRP水平有关。血清NT-proBNP、CRP水平上升与PA水平下降预示着川崎病患儿心血管病变易感性提升。可以将NT-proBNP及PA水平作为预测川崎病患儿是否发生心血管病变的血清学指标。
Abstract:
Objective:To explore the correlation between cardiovascular disease susceptibility and serum levels of N-terminal B-type natriuretic peptide(NT-proBNP),prealbumin(PA)and C-reactive protein(CRP)in children with Kawasaki disease.Methods: The clinical data of 92 children with Kawasaki disease were retrospectively analyzed.They were divided into group A(49 cases with cardiovascular disease)and group B(43 cases without cardiovascular disease)according to the presence or absence of cardiovascular disease.Another 40 healthy children were selected as control group.Serum levels of NT-proBNP,PA and CRP were compared among the three groups.Pearson correlation analysis was used to analyze the correlation between cardiovascular disease and serum levels of NT-proBNP,PA and CRP in children with Kawasaki disease.Logistic regression model was used to analyze the risk factors of cardiovascular disease in children with Kawasaki disease.Results:There were significant differences in serum NT-proBNP,PA and CRP levels among the three groups(P<0.05).The levels of serum NT-proBNP and CRP in group A and group B were higher than those in control group,and those in group A were higher than those in group B(all P<0.05).Serum PA level in group A and group B was lower than that in control group,and that in group A was lower than that in group B(all P<0.05).Pearson correlation analysis showed that cardiovascular disease in children with Kawasaki disease was positively correlated with serum NT-proBNP and CRP levels,and negatively correlated with serum PA levels(r=0.463,0.356,-0.391,all P<0.05).Logistic regression analysis showed that serum NT-proBNP and PA levels were independent risk factors for cardiovascular disease in children with Kawasaki disease(all P<0.05).Conclusion:Cardiovascular disease susceptibility in children with Kawasaki disease is related to serum NT-proBNP,PA and CRP levels.Elevated serum NT-proBNP and CRP levels and decreased PA level indicate increased cardiovascular disease susceptibility in children with Kawasaki disease.NT-proBNP and PA levels can be used as serological indicators to predict the development of cardiovascular disease in children with Kawasaki disease.
