摘要:
目的:探讨布地奈德联合持续正压通气治疗新生儿呼吸窘迫综合征的有效性和安全性。方法:选取新生儿呼吸窘迫综合征患儿88例,以随机数字表法分成对照组和观察组,每组各44例。对照组患儿常规治疗结合持续正压通气治疗,观察组患儿在对照组治疗的基础上再以布地奈德雾化吸入治疗,观察两组患儿治疗前后的治疗效果、血气指标、炎性因子水平及氧疗时间、正压通气时间、住院时间。结果:观察组患儿治疗总有效率(93.18%)明显高于对照组患儿治疗总有效率(75.00%)(P<0.05); 治疗前,两组患儿动脉血氧饱和度(SaO2)、二氧化碳分压(PaCO2)、血氧分压(PaO2)指标对比差异无统计学意义(P>0.05); 治疗后,两组患儿SaO2、PaCO2、PaO2指标对比差异有统计学意义(均P<0.05); 较治疗前,治疗后两组患儿SaO2、PaO2指标水平均明显升高,PaCO2指标水平均降低(均P<0.05); 治疗前,两组患儿炎性因子白介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平比较差异无统计学意义(P>0.05); 治疗后,两组患儿的炎性因子IL-6、TNF-α水平均与治疗前比较降低(均P<0.05); 治疗后,观察组患儿的炎性因子IL-6、TNF-α水平明显低于对照组(均P<0.05); 观察组患儿住院费用、住院时间、正压通气时间、氧疗时间明显少于对照组(均P<0.05); 治疗后,观察组患儿并发症的总发生率(15.91%)明显低于对照组(2.27%)(P<0.05)。结论:布地奈德联合持续正压通气治疗新生儿呼吸窘迫综合征效果显著,能明显缩短患儿住院时间和正压通气时间,减轻患儿家庭的医疗负担,并降低其炎性因子水平,同时也能改善其血气指标、降低并发症的发生率、安全可靠。
Abstract:
Objective:To investigate the efficacy and safety of budesonide combined with continuous positive pressure ventilation in the treatment of neonatal respiratory distress syndrome.Methods: A total of 88 infants with neonatal respiratory distress syndrome were selected and randomly divided into control group and observation group,with 44 cases in each group.The children in control group were treated with conventional treatment combined with continuous positive pressure ventilation.The children in observation group were treated with budesonide nebulized inhalation on the basis of control group.The therapeutic effect,blood gas indexes,inflammatory factor levels,oxygen therapy time,positive pressure ventilation time and hospitalization time of the two groups were observed before and after treatment.Results:The total effective rate in observation group was significantly higher than that in control group(93.18% vs.75.00%,P<0.05).Before treatment,there was no significant difference in arterial oxygen saturation(SaO2),partial pressure of carbon dioxide(PaCO2)and partial pressure of blood oxygen(PaO2)between the two groups(all P>0.05).After treatment,there were significant differences in SaO2,PaCO2 and PaO2 between the two groups(all P<0.05).Compared with before treatment,the levels of SaO2 and PaO2 in both groups were significantly increased,and PaCO2 decreased after treatment(all P<0.05).here was no significant difference in the levels of inflammatory cytokines interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)between the two groups(all P>0.05).After treatment,the levels of IL-6 and TNF-α in the two groups were lower than those before treatment,and the levels of and TNF-α in observation group were significantly lower than those in ontrol group(all P<0.05).The hospitalization cost,hospitalization time,positive pressure ventilation time and oxygen therapy time in bservation group were significantly lower than those in ontrol group(all P<0.05).After treatment,the total incidence of complications of observation group was significantly lower than that of control group(15.91% vs.2.27%,P<0.05).Conclusion:The combination of budesonide and continuous positive pressure ventilation in the treatment of NRDS has a significant effect,which can significantly shorten the hospitalization time and positive pressure ventilation time of the children,reduce the medical burden of the children's family,reduce the level of inflammation,and also improve the blood gas indexes,reduce the incidence of complications,and is safe and reliable.
