摘要:
目的:探讨血清趋化因子、Clara细胞蛋白-16(CC-16)表达水平与急性呼吸窘迫综合征(ARDS)严重程度的相关性。方法:选择80例ARDS患者,根据氧合指数将其分为轻度组(n=25)、中度组(n=34)和重度组(n=21),检测血清趋化因子白细胞介素(IL)-6、IL-8及CC-16水平,分析各参数之间的相关性,绘制受试者操作特征曲线(ROC)判断各指标对重度ARDS的预测效能。结果:IL-6、IL-8、CC-16随着ARDS 病情的加重而升高,三组血清IL-6、IL-8、CC-16比较差异有统计学意义(均P<0.05)。相关性分析显示,血清IL-6、IL-8、CC-16与氧合指数均呈中等强度相关(r=0.472、0.439、0.433),IL-6与IL-8呈中等强度相关(r=0.582),IL-6与ICU停留天数呈弱相关(r=0.302)。血清IL-6、IL-8、CC-16对ARDS病情严重程度均具有预测价值,预测重度ARDS的曲线下面积(AUC)分别为0.804、0.716、0.722,敏感度分别为80.3%、76.4%、78.3%,特异度分别为84.0%、78.1%、77.5%,阳性预测值分别为83.8%、76.9%、80.5%,阴性预测值分别为70.2%、74.8%、68.3%。结论:血清趋化因子IL-6、IL-8、CC-16对判断ARDS 严重程度具有潜在价值。
Abstract:
Objective:To investigate the correlation between the expression levels of serum chemokines,Clara cell protein-16(CC-16)and the severity of acute respiratory distress syndrome(ARDS).Methods: A total of 80 patients with ARDS were selected and divided into mild group(25 cases),moderate group(34 cases)and severe group(21 cases)according to oxygenation index.The levels of serum chemokines interleukin(IL)-6,IL-8 and CC-16 were measured.The correlation between the parameters was analyzed,and the receiver operating characteristic(ROC)curve was drawn to judge the prediction efficiency of each index for severe ARDS.Results:IL-6,IL-8 and CC-16 increased with the aggravation of ARDS.There was significant difference in serum IL-6,IL-8 and CC-16 among the three groups(P<0.05).Correlation analysis showed that serum IL-6,IL-8 and CC-16 were moderately correlated with oxygenation index(r=0.472,0.439 and 0.433),and IL-6 was moderately correlated with IL-8(r=0.582),and IL-6 was weakly correlated with ICU stay days(r=0.302).Serum IL-6,IL-8 and CC-16 had predictive value for the severity of ARDS.The area under the curve(AUC)for predicting severe ARDS were 0.804,0.716 and 0.722,the sensitivity were 80.3%,76.4% and 78.3%,the specificity were 84.0%,78.1% and 77.5%,the positive predictive values were 83.8%,76.9% and 80.5%,and the negative predictive values were 70.2%,74.8% and 68.3%,respectively.Conclusion:Serum chemokines IL-6,IL-8 and CC-16 have potential value in judging the severity of ARDS.
