摘要:
目的:探索气管镜下宏基因组学二代测序技术在鉴别诊断肺癌的临床应用价值。方法:选取32例肺癌患者(M组)、20例肺部非肺癌肺部疾病患者(MN组)以及同时期进行体检的18例健康人群(N组)作为研究对象。采用宏基因组学二代测序技术分析三组研究对象咽部以及肺泡灌洗液的菌群特征,比较三组研究对象微生物菌群的物种丰度差异; 同时通过对研究对象进行代谢组学分析,以找出临床上肺癌患者确诊的潜在标志物。结果:对于咽部微量菌群而言,M组患者Shannon以及Simpson指数明显低于N组、MN组(均P<0.05); 常见的菌门分别为拟杆菌门、厚壁菌门、变形菌门与梭杆菌门,N组患者厚壁菌门与梭杆菌门丰度与M组、MN组比较差异有统计学意义(均P<0.05)。对于肺泡灌洗液微量菌群而言,N组患者Shannon以及Simpson指数明显高于M组、MN组(均P<0.05); 常见的菌门分别为拟杆菌门、厚壁菌门、变形菌门、梭杆菌门、放线菌门与螺杆菌门,N组患者厚壁菌门与梭杆菌门丰度与M组、MN组比较具有统计学差异(均P<0.05),MN组患者放线菌门丰度与M组、N组比较具有统计学差异(均P<0.05)。经过筛选和挖掘三组间菌群存在差异且找出三个潜在标志物分别是葡萄球菌属、链球菌属和硒单胞菌属,根据研究结果表明,这三种菌属诊断鉴别肺癌的价值最高。其中他们的准确度分别是0.93、0.88、0.66; 精度分别是0.88、0.75、0.63。结论:宏基因组学二代测序技术用来预测诊断肺癌是有一定运用价值的。葡萄球菌属、链球菌属和硒单胞菌属可作为肺癌诊断的潜在标志物。
