摘要:
目的:研究粪便硫酸类肝素蛋白多糖2(SDC2)与分泌型卷曲相关蛋白2(SFRP2)基因甲基化检测在结直肠癌(CRC)筛查中的应用价值。方法:选取CRC患者51例为CRC组。选取同期进展期腺瘤患者61例为进展期腺瘤组,选取同期健康体检者50例为正常对照组。采集所有受试者粪便样本,以甲基化特异性PCR法检测粪便SDC2及SFRP2基因甲基化情况。比较各组粪便SDC2及SFRP2基因甲基化情况。分析粪便SDC2及SFRP2基因甲基化与CRC患者临床病理特征的关系。采用受试者工作特征(ROC)曲线分析粪便SDC2及SFRP2基因甲基化检测筛查CRC的效能。结果:CRC组患者粪便SDC2及SFRP2基因甲基化率高于进展期腺瘤组和正常对照组,且进展期腺瘤组粪便SDC2及SFRP2基因甲基化率高于正常对照组(均P<0.05)。TNM分期Ⅲ-Ⅳ期CRC患者粪便SDC2及SFRP2基因甲基化率高于Ⅰ-Ⅱ期患者(均P<0.05)。而性别、年龄、病变部位、肿瘤大小以及分化程度均与CRC患者粪便SDC2及SFRP2基因甲基化无相关性(均P>0.05)。ROC曲线分析显示,粪便SDC2及SFRP2基因甲基化单项检测筛查CRC的效能较高,且两者联合检测的效能高于单项检测。结论:粪便SDC2及SFRP2基因甲基化检测有助于筛查CRC,两者联合检测的筛查效能更高。
Abstract:
Objective:To study the application value of feces detection of syndecan-2(SDC2)and s secreted frizzled related protein 2(SFRP2)gene methylation in colorectal cancer(CRC)screening.Methods:51 patients with CRC were selected as CRC group,and 61 patients with advanced adenoma in the same period were selected as the advanced adenoma group,and 50 healthy people in the same period were selected as the normal control group.The feces samples of all subjects were collected and the methylation of SDC2 and SFRP2 genes were detected by methylation specific PCR.The methylation of SDC2 and SFRP2 genes in feces of each group were compared.The relationship between feces SDC2 and SFRP2 gene methylation and clinicopathological features of CRC patients was analyzed.Receiver operating characteristic(ROC)curve was used to analyze the efficacy of feces SDC2 and SFRP2 gene methylation detection in screening CRC.Results:The methylation rates of feces SDC2 and SFRP2 genes in CRC group were higher than those in advanced adenoma group and normal control group,and the methylation rates of feces SDC2 and SFRP2 genes in advanced adenoma group were higher than those in normal control group(all P<0.05).The methylation rates of feces SDC2 and SFRP2 genes in CRC patients with TNM stage Ⅲ to Ⅳ were higher than those in CRC patients with stage Ⅰ to Ⅱ(all P<0.05).Gender,age,lesion location,tumor size and degree of differentiation were not correlated with feces SDC2 and SFRP2 gene methylation in CRC patients(all P>0.05).ROC curve analysis showed that the single detection of feces SDC2 and SFRP2 gene methylation had higher efficiency in screening CRC,and the efficiency of combined detection was higher than that of single detection.Conclusion:The detection of feces SDC2 and SFRP2 gene methylation is helpful to screen CRC,and the combined detection of them is more effective.
