摘要:
目的:探讨急性痛风性关节炎(AGA)患者肌骨超声半定量评分与红细胞沉降率(ESR)、白细胞介素-6(IL-6)水平及疾病活动度的相关性。方法:选取AGA患者72例,根据功能失调性态度问卷(DAS)28评分分为中高活动期组(35例)和低活动期组(37例)。比较两组患者肌骨超声半定量评分、ESR、IL-6、DAS28评分。分析肌骨超声半定量评分与ESR、IL-6、DAS28评分的相关性。采用受试者工作特征(ROC)曲线分析肌骨超声半定量评分、ESR、IL-6对AGA疾病活动度的评估价值。结果:中高活动期组滑膜增生评分、血流信号评分、关节积液评分、骨侵蚀评分与总评分高于低活动期组,ESR、IL-6、DAS28评分也高于活动期组(均P<0.05)。Pearson相关性分析显示,AGA患者肌骨超声半定量评分与ESR水平、DAS28评分呈正相关(均P<0.05); AGA患者肌骨超声半定量评分中的滑膜增生评分、关节积液评分、总评分与IL-6水平呈正相关(均P<0.05)。ROC曲线分析显示,肌骨超声半定量评分及ESR、IL-6对AGA疾病活动度的评估价值较理想,曲线下面积均>0.75。结论:AGA患者肌骨超声半定量评分与ESR、IL-6、疾病活动度均具有相关性,可以作为AGA疾病活动度的良好评估手段。
Abstract:
Objective:To investigate the correlation between semi-quantitative score of musculoskeletal ultrasound and erythrocyte sedimentation rate(ESR),interleukin-6(IL-6),disease activity in patients with acute gouty arthritis(AGA).Methods:72 patients with AGA were selected and divided into middle-high activity group(35 cases)and low activity group(37 cases)according to DAS 28 score.All patients underwent musculoskeletal ultrasound examination.The semi-quantitative score of musculoskeletal ultrasound,ESR,IL-6 and DAS28 score were compared between the two groups.The correlation between semi-quantitative score of musculoskeletal ultrasound and ESR,IL-6,DAS28 score was analyzed.Receiver operating characteristic(ROC)curve was used to analyze the evaluation value of semi-quantitative score of musculoskeletal ultrasound,ESR and IL-6 on AGA disease activity.Results:The synovial membrane hyperplasia score,blood flow signal score,joint effusion score,bone erosion score and total score in the middle-high activity group were higher than those in the low activity group,and the ESR,IL-6 and DAS28 score were also higher than those in the low activity group(all P<0.05).Pearson correlation analysis showed that the semi-quantitative score of musculoskeletal ultrasound in AGA patients was positively correlated with ESR level and DAS28 score(all P<0.05); the synovial membrane hyperplasia score,joint effusion score and total score in AGA patients were positively correlated with the level of IL-6(all P<0.05).ROC curve analysis showed that the semi-quantitative score of musculoskeletal ultrasound,ESR and IL-6 had ideal value in evaluating AGA disease activity,and the area under the curve were all more than 0.75.Conclusion:The semi-quantitative score of musculoskeletal ultrasound is correlated with ESR,IL-6 and disease activity,which can be used as good assessment tool to assess disease activity of AGA.
