摘要:
目的:采用超声生物显微镜(UBM)测量角膜水平直径,比较其测量结果与传统规尺法的差异性,探讨有晶状体眼后房型人工晶状体(ICL)植入术的有效性和安全性。方法:选取双眼ICL植入术患者126例252眼,将所有患者随机分为两组,对照组和观察组,各63例,各126眼。对照组接受规尺法测量角膜水平直径并行ICL植入术,观察组接受UBM测量角膜水平直径并行ICL植入术。记录并比较两组手术前和手术后1、3个月的眼科参数,包括裸眼视力、眼压、前房角、对比敏感度、高阶像差、残留屈光度(等值球径、球径、柱径)、安全指数、疗效指数等。结果:与对照组比,观察组的角膜水平直径明显增高(P<0.05); 术前,两组患者3点与9点方向前房角比较差异无统计学意义(均P>0.05),术后1、3个月,两组患者3点与9点方向前房角均明显减少(均P<0.05),与对照组相比,观察组患者3点与9点方向前房角均明显减少(均P<0.05); 两组患者手术前后的眼压及高阶像差比较差异无统计学意义(均P>0.05); 与术前比,两组患者术后1、3个月的裸眼视力明显增高(均P<0.05),与对照组比,观察组术后1、3个月的裸眼视力明显增高(均P<0.05); 两组患者手术前明视和暗视的敏感度比较差异无统计学意义(均P>0.05),与对照组比,观察组术后1、3个月的明视和暗视的敏感度均有不同程度地增高(均P<0.05); 两组患者手术前后残留屈光度比较差异无统计学意义(P>0.05); 与术后1个月比较,两组术后3个月的疗效指数和安全指数均明显提高(均P<0.05),与对照组比较,观察组的疗效指数和安全指数明显增高(均P<0.05)。结论:UBM在ICL植入手术术前测量角膜水平直径具有明显的临床优势,有助于指导ICL植入手术术前选择合适的晶状体,提高ICL植入术的有效性和安全性。
Abstract:
Objective:To measure the horizontal diameter of the cornea with an ultrasound biomicroscopy(UBM),and to compare the difference between the UBM and the traditional ruler method,and to explore the implantable collamer lens(ICL)implantation in phakic eyes effectiveness and safety.Methods:A total of 252 eyes of 126 patients with ICL implantation were selected.All patients were randomly divided into control group and observation group,with 63 cases in each group,126 eyes in each group.The control group received the measurement of the horizontal diameter of the cornea by the ruler method and ICL implantation,and the observation group received the measurement of the horizontal diameter of the cornea by UBM and the ICL implantation.The ophthalmic parameters before the operation and 1 and 3 months after the operation,including naked vision,intraocular pressure,anterior chamber angle,contrast sensitivity,high-order aberrations,residual refractive power(equivalent spherical diameter,spherical diameter,cylindrical diameter),safety index,efficacy index,etc.were recorded and compared.Results:Compared with the control group,the horizontal corneal diameter of the observation group was significantly higher(P<0.05).Before the operation,there was no significant difference in the anterior chamber angle between the 3 o'clock and 9 o'clock directions between two groups(all P>0.05).At 1 month and 3 months after the operation,the 3 o'clock and 9 o'clock anterior chamber angles of two groups were significantly reduced(all P<0.05).Compared with the control group,the anterior chamber angles at 3 o'clock and 9 o'clock in the observation group were significantly reduced(all P<0.05).There was no statistical difference in intraocular pressure and higher-order aberrations between two groups before and after surgery(P>0.05).Compared with preoperatively,the naked eyesight of two groups was significantly increased at 1 month and 3 months after surgery(all P<0.05).Compared with the control group,the uncorrected visual acuity 1 month and 3 months after surgery in the observation group was significantly increased(all P<0.05).There was no statistically significant difference in the sensitivity of photopic and scotopic vision between two groups before surgery(P>0.05).Compared with the control group,the sensitivity of both photopic and scotopic vision in the observation group at 3 months was increased(all P<0.05).There was no statistically significant difference in residual refractive power between 2 groups before and after surgery(P>0.05).Compared with 1 month after surgery,the efficacy index and safety index of two groups after 3 months were significantly improved(all P<0.05).Compared with the control group,the efficacy index and safety index of the observation group were significantly higher(all P<0.05).Conclusion:UBM has obvious clinical advantages in measuring the horizontal diameter of the cornea before ICL implantation surgery,which is helpful to guide the selection of the appropriate lens before ICL implantation surgery,and improve the effectiveness and safety of ICL implantation.
