摘要:
目的:探讨α-突触核蛋白与帕金森病患者伴发快速眼动期睡眠行为障碍(RBD)的相关性。方法:根据REM睡眠行为异常问卷评分结果将70例帕金森患者分为帕金森病患者伴发RBD组(PD+RBD组,n=26)、帕金森病患者不伴RBD组(PD-RBD组,n=44)。测定两组受试者血浆及唾液中α-突触核蛋白含量并进行比较; 采用Epworth 嗜睡评分量表(ESS)评估两组受试者睡眠质量,分别采用汉密尔顿抑郁量表(HAMD)、汉密尔顿焦虑量表(HAMA)评估两组受试者抑郁、焦虑状况,采用统一帕金森病量表Ⅲ(UPDRS Ⅲ)、Hoehn-Yahr(H-Y)分期评价两组受试者的运动能力; 采用Logistic多因素回归分析帕金森患者伴发RBD的相关因素。结果:PD+RBD组患者病程明显长于PD-RBD组(P<0.05)。PD+RBD组患者血浆及唾液中α-突触核蛋白含量均高于PD-RBD组(P<0.05)。PD+RBD组患者ESS评分高于PD-RBD组(P<0.05),HAMD评分、HAMA评分高于PD-RBD组(均P<0.05)。两组受试者UPDRS Ⅲ评分比较无统计学差异(P>0.05),PD+RBD组受试者H-Y分期高于PD-RBD组(P<0.05)。Logistic多因素回归分析结果显示,病程、α-突触核蛋白含量与帕金森病患者伴发RBD显著相关(均P<0.05)。结论:血浆及唾液中α-突触核蛋白表达量升高是帕金森病患者伴发RBD的独立影响因素。
Abstract:
Objective:To investigate the relationship between α-synuclein and rapid eye movement(REM)sleep behavior disorder(RBD)in Parkinson's disease.Methods:According to the results of REM sleep behavior abnormality questionnaire,70 patients with Parkinson's disease were divided into Parkinson's disease patients with RBD group(PD+RBD group,26 cases)and Parkinson's disease patients without RBD group(PD-RBD group,44 cases).The contents of α-synuclein in plasma and saliva were measured and compared.Epworth Sleepiness Scale(ESS)was used to evaluate the sleep quality of the two groups.Hamilton Depression Scale(HAMD)and Hamilton Anxiety Scale(HAMA)were used to evaluate the depression and anxiety of the two groups.Unified Parkinson's Disease Scale Ⅲ(UPDRS Ⅲ)and Hoehn-Yahr(H-Y)staging were used to evaluate the motor ability of the two groups.Logistic multivariate regression was used to analyze the related factors of RBD in patients with Parkinson's disease.Results:The course of disease in PD+RBD group was significantly longer than that in PD-RBD group(P<0.05).The content of α-synuclein in plasma and saliva of patients in PD+RBD group was higher than that in PD-RBD group(all P<0.05).ESS score,AMD score and HAMA score in PD+RBD group were higher than those in PD-RBD group(all P<0.05).There was no significant difference in UPDRS Ⅲ score between the two groups(P>0.05).The H-Y staging in PD+RBD group was higher than that in PD-RBD group(P<0.05).Logistic multivariate regression analysis showed that the course of disease and the content of α-synuclein were significantly correlated with RBD in patients with Parkinson's disease(all P<0.05).Conclusion:Increased expression of α-synuclein in plasma and saliva is an independent risk factor for RBD in patients with Parkinson's disease.
