摘要:
目的:探讨子宫肌瘤患者凝血功能与子宫体积的关系及其对继发性贫血的预测价值。方法:选择180例子宫肌瘤患者作为观察组,另选同期的180例健康体检者作为对照组。比较两组凝血功能的差异,使用Pearson相关性分析凝血功能指标与子宫体积的关系,以受试者工作特征曲线(ROC)下面积(AUC)评价凝血功能指标对继发性贫血的预测效能。结果:观察组子宫体积大于对照组,差异有统计学意义(P<0.05); 观察组活化部分凝血活酶时间(APTT)、凝血酶原时间(PT)长于对照组,凝血酶时间(TT)短于对照组,纤维蛋白原(FIB)水平高于对照组,差异均有统计学意义(均P<0.05); 经Pearson相关性分析,子宫肌瘤患者PT、FIB均与子宫体积呈正相关(r=0.264、0.278,P=0.016、0.014); 在观察组180例子宫肌瘤患者中,继发性贫血发生率为26.11%,其中贫血者APTT长于非贫血者,FIB水平高于非贫血者,差异均有统计学意义(均P<0.05); 经ROC曲线分析,APTT联合FIB预测子宫肌瘤患者发生继发性贫血的AUC为0.904。结论:子宫肌瘤患者存在凝血功能异常,PT、FIB与子宫体积呈正相关,APTT联合FIB预测继发性贫血的效能较好,值得临床予以重视。
Abstract:
Objective:To analyze the relationship between coagulation function and uterine volume in patients with uterine fibroids and its predictive value for secondary anemia.Methods:A total of 180 patients with uterine fibroids were selected as the observation group,and another 180 healthy subjects during the same period were selected as the control group.The difference of coagulation function between the two groups was compared,and Pearson correlation was used to analyze the relationship between coagulation function indexes and uterine volume.The predictive effect of coagulation function indexes for secondary anemia was evaluated by receiver operating characteristic(ROC)curve.Results:The uterine volume of the observation group was larger than that of the control group(P<0.05).Activated partial thromboplastin time(APTT)and prothrombin time(PT)in the observation group were longer than those in the control group,and thrombin time(TT)was shorter than that in the control group,and fibrinogen(FIB)level was higher than that in the control group(all P<0.05).Pearson correlation analysis showed that PT and FIB were positively correlated with uterine volume(r=0.264,0.278,P=0.016,0.014).In the observation group,the incidence of secondary anemia was 26.11%,and the APTT of anemia patients was longer than that of non-anemia patients,and the FIB level of anemia patients was higher than that of non-anemia patients(all P<0.05).By ROC curve analysis,the area under curve(AUC)of APTT combined with FIB in predicting secondary anemia in patients with uterine fibroids was 0.904.Conclusion:There is abnormal coagulation function in patients with uterine fibroids,PT and FIB are positively correlated with uterine volume,APTT combined with FIB is effective in predicting secondary anemia,which deserves clinical attention.
