摘要:
目的:分析乳腺癌患者病灶血流时间-信号强度曲线(TIC)类型、表观弥散系数(ADC)与化疗疗效的相关性。方法:选择乳腺癌患者172例作为研究对象,所有患者都给予全身化疗治疗。在化疗前与化疗后3个月进行磁共振成像(MRI)检查,比较化疗前后TIC类型、ADC值,并与化疗疗效进行相关性分析。结果:化疗前患者的病灶血流TIC类型多为Ⅲ型,化疗3个月后多为Ⅰ型和Ⅱ型,化疗前后TIC类型比较差异有统计学意义(P<0.05)。取MRI DWI b值为500、1000 s/mm2,化疗3个月后患者病灶ADC值高于化疗前(均P<0.05)。所有患者完成化疗,172例患者中完全缓解89例,部分缓解41例,稳定30例,进展12例,总有效率为75.6%。Spearman相关性分析显示,乳腺癌患者病灶血流TIC类型、ADC值与化疗疗效存在相关性(均P<0.05)。Logistic回归分析显示,病灶血流TIC类型、ADC值为乳腺癌患者化疗疗效的影响因素(均P<0.05)。结论:乳腺癌患者病灶血流TIC类型、ADC值与化疗疗效存在相关性。MRI能为患者疗效评估提供更为可靠的影像学依据。
Abstract:
Objective:To analyze the correlation between the type of focal blood flow time-signal intensity curve(TIC),apparent diffusion coefficient(ADC)and the efficacy of chemotherapy in breast cancer patients.Methods:172 patients with breast cancer were selected as the research objects,and all patients were treated with systemic chemotherapy.MRI examinations were performed before and 3 months after chemotherapy.The TIC type and ADC value before and after chemotherapy were compared,and the correlation between them and the efficacy of chemotherapy was analyzed.Results:The TIC type of the focal blood flow before chemotherapy was mostly type Ⅲ,and most of them were type Ⅰ and type Ⅱ after 3 months of chemotherapy.The difference in TIC types before and after chemotherapy was statistically significant(P<0.05).As MRI DWI b values were 500 and 1000 s/mm2,the focal ADC values after 3 months of chemotherapy were higher than those before chemotherapy(all P<0.05).All patients completed chemotherapy.Of the 172 patients,89 were in complete remission,41 were in partial remission,30 were stable,12 were progressing,and the total effective rate was 75.6%.Spearman correlation analysis showed that there was a correlation between TIC type of the blood flow,ADC value and chemotherapy efficacy in breast cancer patients(all P<0.05).Logistic regression analysis showed that the TIC type of the blood flow,ADC value were factors influencing the chemotherapy efficacy of breast cancer patients(both P<0.05).Conclusion:There was a correlation between TIC type of the blood flow,ADC value and chemotherapy efficacy in breast cancer patients.MRI can provide a more reliable imaging basis for efficacy evaluation.
