摘要:
目的:研究高压氧(HBO)联合持续正压通气(CPAP)治疗重度阻塞性睡眠呼吸暂停低通气综合征(OSAHS)疗效及对患者睡眠结构的影响。方法:重度OSAHS患者150例,根据治疗方法的不同分为CPAP组、HBO组和联合组,各50例。CPAP组使用CPAP治疗,HBO组使用HBO进行治疗,联合组联合应用CPAP和HBO进行治疗。对三组治疗后打鼾、晨起口干以及夜间憋醒的发生率进行比较。对三组治疗前后的睡眠状态指标和睡眠结构进行分析。比较三组的治疗安全性。结果:治疗后,联合组打鼾、晨起口干和夜间憋醒的发生率均明显低于CPAP组和HBO组(均P<0.05)。治疗后,联合组的Epworth睡眠评分(ESS评分)、呼吸暂停低通气指数(AHI)水平均明显低于CPAP组和HBO组,动脉血氧饱和度(SaO2)水平明显高于CPAP组和HBO组(均P<0.05)。治疗后,联合组的睡眠转换次数、Ⅰ期+Ⅱ期占比均明显低于CPAP组和HBO组(P<0.05),Ⅲ期+Ⅳ期、快速眼动睡眠(REM)期占比均明显高于CPAP组和HBO组(均P<0.05)。三组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:HBO联合CPAP治疗重度OSAHS,能够明显减轻患者的临床症状,改善睡眠状态和睡眠结构,且安全性好。
Abstract:
Objective:To study the efficacy of hyperbaricoxygen(HBO)and continuous positive pressure ventilation(CPAP)in the treatment of severe obstructive sleep apnea-hypopnea syndrome(OSAHS)and its influence on the sleep structure of patients.Methods:According to different treatment methods,150 cases of severe OSAHS patients were divided into CPAP group,HBO group and combination group,with 50 cases in each.CPAP group was treated with CPAP,HBO group was treated with HBO,and the combination group was treated with CPAP and HBO.The incidences of snoring,dry mouth in the morning and waking up at night were compared among the three groups after treatment.The sleep state indicators and sleep structure of the three groups before and after treatment were analyzed.The treatment safety of the three groups was compared.Results:After treatment,the incidences of snoring,dry mouth in the morning and waking up at night in the combination group were significantly lower than those in the CPAP group and the HBO group(all P<0.05).After treatment,the ESS score and AHI of the combination group were significantly lower than those of the CPAP group and HBO group,and the SaO2 level was significantly higher than that of the CPAP group and HBO group(allP<0.05).After treatment,the level of sleep conversions,proportions of stage Ⅰ+Ⅱ in the combination group were significantly lower than those in the CPAP group and HBO group,and the proportions of stage Ⅲ+Ⅳ and rapid eye movement(REM)stage were significantly higher than those in the CPAP group and HBO group(all P<0.05).There was no statistically significant difference in the incidence of adverse reactions among the three groups(P>0.05).Conclusion:HBO combined with CPAP in the treatment of severe OSAHS can significantly reduce the clinical symptoms of patients,improve sleep state and sleep structure,and has good safety.
