摘要:
目的:研究了维持性血液透析(MHD)患者骨硬化蛋白(SCL)、Klotho蛋白水平变化及与冠状动脉钙化的相关性。方法:纳入收治的MHD患者180例,记录患者的冠状动脉钙化积分(CACs)。分析不同冠状动脉钙化程度患者的临床资料,使用二元Logistic回归对MHD患者冠状动脉钙化发生的危险因素进行分析。使用ROC曲线分析SCL、Klotho蛋白在MHD患者冠状动脉钙化诊断中的应用价值。结果:不同冠状动脉钙化程度患者之间年龄、糖尿病、SCL、Klotho蛋白以及25羟维生素D3水平比较,差异具有统计学意义(均P<0.05)。年龄≥60岁、SCL为MHD患者发生冠状动脉钙化的危险因素,Klotho蛋白为MHD患者发生冠状动脉钙化的保护性因素。SCL对MHD患者冠状动脉钙化诊断下,AUC为0.711,诊断敏感度为61.50%,特异度为75.56%。Klotho蛋白对MHD患者冠状动脉钙化诊断下,AUC为0.802,诊断敏感度为82.20%,特异度为62.96%。结论: SCL、Klotho蛋白水平均与MHD患者冠状动脉钙化密切相关,且SCL、Klotho蛋白水平对MHD患者冠状动脉钙化具有较好的诊断价值。临床中可据此为MHD患者冠状动脉钙化的早期诊断及干预制定相关措施,以降低MHD患者心血管不良事件的发生风险。
Abstract:
Objective:To study the changes of bone sclerostin(SCL)and Klotho protein levels in maintenance hemodialysis(MHD)patients and their correlation with coronary artery calcification.Methods:180 cases of MHD patients were enrolled,and the CACs of the patients were recorded.The clinical data of patients with different degrees of coronary artery calcification were analyzed,and the risk factors of coronary artery calcification in MHD patients were analyzed by using binary Logistic regression.The ROC curve was used to analyze the application value of SCL and Klotho protein in the diagnosis of coronary artery calcification in MHD patients.Results:The differences in age,diabetes,SCL,Klotho protein,and 25-hydroxyvitamin D3 levels between patients with different coronary artery calcification levels were statistically significant(all P<0.05).Age ≥ 60 years old and SCL were risk factors for coronary artery calcification in MHD patients,and Klotho protein was protective factor for coronary artery calcification in MHD patients.In the diagnosis of coronary artery calcification by SCL in MHD patients,the AUC was 0.711,the sensitivity was 61.50%,and the specificity was 75.56%.In the diagnosis of coronary artery calcification by Klotho protein in MHD patients,the AUC was 0.802,the sensitivity was 82.20%,and the specificity was 62.96%.Conclusion:SCL and Klotho protein levels are closely related to coronary artery calcification in MHD patients,and SCL and Klotho protein levels have good diagnostic value for coronary artery calcification in MHD patients.Based on this,relevant measures can be formulated for early diagnosis and intervention of coronary artery calcification in MHD patients in clinical practice,so as to reduce the risk of adverse cardiovascular events in MHD patients.
