摘要:
目的:研究慢性阻塞性肺疾病急性加重期(AECOPD)患者干扰素-γ(INF-γ)、甲壳质酶蛋白-40(YKL-40)水平及与预后、肺功能的相关性。方法:纳入AECOPD患者140例作为AECOPD组,COPD稳定期患者90例作为稳定组,同期来院体检的健康志愿者90例作为对照组。对三组INF-γ、YKL-40水平及肺功能指标进行比较。使用Pearson检验分析稳定组和AECOPD组患者INF-γ、YKL-40与肺功能的相关性。将AECOPD 组患者分为预后不良组和预后良好组,比较两组的INF-γ、YKL-40水平。使用Spearman检验分析AECOPD组患者INF-γ、YKL-40水平与预后的相关性。使用ROC曲线分析INF-γ、YKL-40对AECOPD患者预后的评估价值。结果:AECOPD组的INF-γ、YKL-40水平明显高于稳定组和对照组(均P<0.05),一秒用力呼吸量(FEV1)、用力肺活量(FVC)水平明显低于稳定组和对照组(均P<0.05)。稳定组和AECOPD组患者INF-γ、YKL-40水平与FEV1、FVC呈明显负相关(均P<0.05)。预后不良组的INF-γ、YKL-40水平明显高于预后良好组(均P<0.05)。AECOPD组患者INF-γ、YKL-40水平与预后呈明显负相关(r分别为-0.406、-0.580,均P<0.05)。INF-γ、YKL-40对AECOPD的诊断中,AUC分别为0.751(95%CI: 0.657~0.845),0.858(95%CI:0.788~0.929)。结论:INF-γ、YKL-40水平在AECOPD患者中明显增高,与患者肺功能、预后呈明显负相关,并可作为AECOPD患者预后判断的可靠指标。
Abstract:
Objective:To study the levels of interferon-γ(INF-γ)and YKL-40 in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD)and their correlation with prognosis and lung function.Methods:A total of 140 AECOPD patients were included as the AECOPD group,90 patients with stable COPD as the stable group,and 90 healthy volunteers for physical examination during the same period served as the control group.The levels of INF-γ,YKL-40 and lung function indexes of the three groups were compared.Pearson test was used to analyze the correlation between INF-γ,YKL-40 and lung function in stable group and AECOPD group.The patients in the AECOPD group were divided into poor prognosis group and good prognosis group,and the levels of INF-γ and YKL-40 were compared between the two groups.Spearman test was used to analyze the correlation between the levels of INF-γ,YKL-40 and the prognosis of patients in the AECOPD group.The ROC curve was used to analyze the evaluation value of INF-γ and YKL-40 on the prognosis of AECOPD patients.Results:The levels of INF-γ and YKL-40 in the AECOPD group were significantly higher than those in the stable group and the control group(all P<0.05),and the levels of FEV1 and FVC were significantly lower than those in the stable group and the control group(all P<0.05).The INF-γ and YKL-40 levels of patients in the stable group and AECOPD group were significantly negatively correlated with FEV1 and FVC(all P<0.05).The levels of INF-γ and YKL-40 in the poor prognosis group were significantly higher than those in the good prognosis group(all P<0.05).The levels of INF-γ and YKL-40 in the AECOPD group were significantly negatively correlated with the prognosis(r=-0.406,-0.580,all P<0.05).In the diagnosis of AECOPD by INF-γ and YKL-40,the AUC were 0.751(95%CI:0.657-0.845)and 0.858(95%CI:0.788-0.929),respectively.Conclusion:The levels of INF-γ and YKL-40 are significantly increased in AECOPD patients,which are significantly negatively correlated with patients' lung function and prognosis,and can be used as reliable indicators for prognostic judgment of AECOPD patients.
