摘要:
目的:探讨过敏性紫癜患儿不同时期血清促炎细胞因子水平变化及检测价值。方法:选取过敏性紫癜患儿65例作为研究组。选取同期进行体检的健康儿童65例作为对照组。对研究组患儿急性期、恢复期以及对照组儿童血清促炎细胞因子[白细胞介素-2(IL-2)、IL-6、IL-33、C反应蛋白(CRP)和肿瘤坏死因子-α(TNF-α)]水平进行检测。比较研究组患儿急性期、恢复期与对照组血清IL-2、IL-6、IL-33、CRP、TNF-α水平变化,分析它们与过敏性紫癜患儿预后的关系。结果:研究组患儿急性期血清IL-2、IL-6、IL-33、CRP、TNF-α水平明显高于对照组(均P<0.05)。研究组患儿恢复期血清IL-2、IL-6、IL-33、CRP、TNF-α水平与对照组比较,差异无统计学意义(均P>0.05)。Logistic回归分析结果显示,血清IL-2、IL-6、IL-33、CRP、TNF-α水平与过敏性紫癜患儿预后相关,在小儿过敏性紫癜发病过程中具有独立作用(均P<0.05)。Pearson相关性分析显示,血清IL-2、IL-6、IL-33、CRP与TNF-α呈正相关(r=0.285、0.596、0.287、0.556,均P<0.001)。结论:血清IL-2、IL-6、IL-33、CRP、TNF-α参与了过敏性紫癜发病及免疫机制,急性期升高,恢复期下降,对患儿病情严重程度及预后评估具有重要价值。
Abstract:
Objective:To investigate the changes of serum pro-inflammatory cytokine levels in children with Henoch-Schonlein purpura in different periods and its detection value.Methods:65 cases of children with Henoch-Schonlein purpura were selected as the study group,and 65 cases of healthy children who underwent health examination in the same period were selected as the control group.The levels of serum pro-inflammatory cytokines [interleukin-2(IL-2),IL-6,IL-33,C-reactive protein(CRP)and tumor necrosis factor- α(TNF-α)] were detected.The changes in serum levels of IL-2,IL-6,IL-33,CRP,and TNF-α during the acute phase and recovery phase in the study group were compared with those in the control group,and their relationship with the prognosis of children with Henoch-Schonlein purpura was analyzed.Results:The levels of serum IL-2,IL-6,IL-33,CRP,and TNF-α during the acute phase in the study group were significantly higher than those in the control group(all P<0.05).The serum levels of IL-2,IL-6,IL-33,CRP and TNF-α during the recovery period in the study group were not significantly different from those in the control group(all P>0.05).Logistic regression analysis showed that serum levels of IL-2,IL-6,IL-33,CRP,and TNF-α were related to the prognosis of children with Henoch-Schonlein purpura,and have an independent role in the pathogenesis of children with Henoch-Schonlein purpura(all P<0.05).Pearson correlation analysis showed that serum IL-2,IL-6,IL-33,CRP were positively correlated with TNF-α(r=0.285,0.596,0.287,0.556,all P<0.001).Conclusion:Serum IL-2,IL-6,IL-33,CRP,and TNF-α participate in the pathogenesis and immune mechanism of Henoch-Schonlein purpura,increase in acute stage and decrease in recover stage,and have important value for the evaluation of the severity of the disease and the prognosis of children.
