摘要:
目的:探讨原发性肝细胞癌(HCC)组织中Foxp3+调节性T淋巴细胞(Foxp3+Treg)表达水平及其与肿瘤免疫的相关性。方法:选取40例HCC组织和40例癌旁组织,应用免疫组化方法检测肝癌组织和癌旁组织中的Foxp3+Treg和CD8+ 细胞毒性T淋巴细胞(CD8+CTL)的表达情况,分析Foxp3+Treg与CD8+ CTL的关系,最后分析肿瘤切除术后Foxp3+Treg表达水平的变化。结果:肝细胞癌组织、癌旁组织Foxp3+Treg、CD8+ CTL表达和Foxp3+Treg/CD8+ CTL比值对比差异有统计学意义(均P<0.01); 不同性别、肿瘤位置、术前AFP水平、肿瘤病理分级的HCC患者Foxp3+Treg水平对比差异无统计学意义(均P>0.05),不同肿瘤大小的Foxp3+Treg水平对比差异有统计学意义(P<0.05); 癌组织中Foxp3+Treg与CD8+ CTL呈负相关(r=0.425,P=0.024),Foxp3+Treg越多,CD8+ CTL越少; 术后3个月进行跟踪监测发现,术后患者外周血中的Foxp3+Treg水平为(5.98±1.34)%,手术前为(9.67±2.25)%,术后明显低于手术前,差异具有统计学意义(P<0.05)。结论:肝细胞癌组织中Foxp3+Treg通过抑制CD8+ CTL的增殖,可抑制其抗肿瘤作用; 通过手术可在一定程度上清除免疫抑制,Foxp3+Treg升高与CD8+ CTL降低可能参与肝肿瘤细胞免疫逃逸,因此可以通过检测Foxp3+Treg与CD8+ CTL来评价HCC患者的细胞免疫功能。
Abstract:
Objective:To investigate the expression level of Foxp3+ regulatory T-lymphocyte(Foxp3+Treg)in hepatocellular carcinoma(HCC)and its correlation with tumor immunity.Methods:40 cases of HCC tissues and 40 cases of adjacent tissues were selected.The immunohistochemistry method was used to detect the expression of Foxp3+ Treg and CD8+ cytotoxic T lymphocytes(CD8+ CTL)in HCC tissues and adjacent tissues.The relationship between Foxp3+ Treg and CD8+ CTL was analyzed.Finally, the changes of Foxp3+ Treg level was analyzed after tumor resection.Results:The expression of Foxp3+ Treg,CD8+ CTL and the ratio of Foxp3+ Treg/CD8+ CTL in HCC tissues and adjacent tissues were statistically different(all P<0.01).There was no significant difference in Foxp3+ Treg level in HCC patients with different gender,tumor location,preoperative AFP level,and tumor pathological grade(all P>0.05).There was statistically significant difference in Foxp3+ Treg level in different tumor size(P<0.05).There was a negative correlation between Foxp3+ Treg and CD8+ CTL in cancer tissue(r=0.425,P=0.024).The more Foxp3+ Treg,the less CD8+ CTL.After 3 months of follow-up monitoring,it was found that the Foxp3+ Treg level in the peripheral blood of patients after surgery was significantly lower than that before surgery [(5.98±1.34)% vs.(9.67±2.25)%,P<0.05].Conclusion:Foxp3+ Treg can inhibit the proliferation of CD8+ CTL in HCC.The immunosuppression can be eliminated to a certain extent by surgery.The increase of Foxp3+ Treg and the decrease of CD8+ CTL may be involved in the immune escape of liver tumor cells.Therefore,the detection of Foxp3+ Treg and CD8+ CTL can be considered to evaluate the cellular immune function of HCC patients.
